Myelodysplastische Syndrome sind eine heterogene Gruppe von Blutstammzellerkrankungen, die vornehmlich bei älteren Patienten auftreten und in eine akute myeloische Leukämie übergehen können. Charakteristisch ist eine mehr oder weniger ausgeprägte hämatopoietische Insuffizienz, welche zu Zytopenie-bedingten Symptomen und Komplikationen führt. Goldstandard für die Diagnosestellung ist weiterhin die zytologische Untersuchung des Knochenmarks. Gemeinsam mit der Zytogenetik bildet sie auch die Grundlage für die Prognosestellung anhand von Risiko-Scores wie dem International Prognostic Scoring System (IPSS) oder seiner überarbeiteten Version (IPSS-R). Die Therapieauswahl richtet sich nach krankheitsspezifischen Faktoren, aber auch nach patientenspezifischen Faktoren wie Alter und Komorbiditäten. Sie reicht von einer rein unterstützenden Therapie wie regelmäßigen Transfusionen, über spezifische Therapie wie Lenalidomid und Azacitidine bis hin zur allogenen Blutstammzelltransplantation als potenziell kurativen Therapieansatz. Die jüngst mithilfe neuer Hochdurchsatzsequenzierungstechnologien identifizierten Genmutationen werden voraussichtlich in Zukunft eine individualisierte Diagnostik, Prognosestellung und Therapie ermöglichen.
Myelodysplastic syndromes (MDS) are a heterogeneous group of blood stem cell disorders, which generally occur in elderly peoples and are associated with the risk to progress towards secondary acute myeloid leukemia. Hematopoietic insufficiency is the hallmark of MDS and causes cytopenia-related symptoms and complications. Cytomorphological investigation of the bone marrow still represents the gold standard to diagnose MDS. Together with cytogenetic analyses it also builds the backbone of established prognostic scores such as the initial International Prognostic Scoring System (IPSS) and its revised version (IPSS-R). The choice of therapy is based on disease-specific, but also patient-specific features such as age and comorbidities. Therapy options range from best supportive care including regular transfusions, specific drugs such as lenalidomide and azacitidine to allogeneic stem cell transplantation as potentially curative approach. The knowledge about gene mutations recently discovered by high-throughpout sequencing technologies will presumably enable a more individualized diagnosis, prognostication and therapy for MDS patients.
