Gas-Alternating-Water Infusion Method: A Novel Technique Of Endoscopic Submucosal Dissection For Colorectal Tumors Regardless Of Gravity

Aim To evaluate the feasibility and safety of ESD using gas-alternating-water infusion method (GAW-ESD) for colorectal laterally spreading tumors.

Method All the patients were kept in left lateral position during GAW-ESD as follows:(1) C-shaped mucosal incision. (2) The colorectal lumen was instilled with normal saline, followed by a V-shaped dissection on both below and above the liquid surface. (3) Post-ESD defect was closed underwater.

Result GAW-ESD was completed without any complications.

Conclusion GAW-ESD is safe and effective for en-bloc resection of colorectal tumors, with the advantages of quick gas/liquid switch, buoyancy effect, heat-sink effect, optical zoom effect, and downsizing effect.
