Akute Schmerztherapie: wissenschaftliche Evidenz - Die aktuelle Leitlinie für Australien und Neuseeland

Seit 1999 veröffentlicht das „Australian and New Zealand College of Anaesthesists“ (ANZCA) in Zusammenarbeit mit seiner Fakultät für Schmerzmedizin (Faculty of Pain Medicine der ANZCA, FPMANZCA) eine evidenzbasierte Leitlinie zur akuten Schmerztherapie mit dem Titel „Acute pain management: scientific evidence“ [1]. Diese wird alle 5 Jahre aktualisiert [2], [3]. Die letzte (vierte) Auflage wurde im Dezember 2015 vorgestellt. Sie ist auf der Website der FPMANZCA (http://fpm.anzca.edu.au/Resources/Publications) als frei verfügbares PDF und seit Mai 2016 auch als Buch erhältlich [4]. Diese Reihe von Veröffentlichungen hat sich in den vergangenen 16 Jahren zu einer weltweit anerkannten Referenz zur akuten Schmerztherapie entwickelt. Auch die jetzt erschienene Ausgabe wurde von zahlreichen nationalen und internationalen Organisationen wie z. B. der IASP formal empfohlen [5].

Fazit Die Evidenz im Bereich der akuten Schmerztherapie hat in den letzten 15 Jahren in beeindruckender Weise zugenommen. Basierend auf den Kernaussagen der 4. Auflage von „Acute Pain Management: Scientific Evidence“ können Ärzte evidenzbasierte Entscheidungen für die Behandlung der meisten akuten Schmerzsituationen treffen. Ein solcher Ansatz verbessert die sichere und wirksame Behandlung bei akuten Schmerzen. Allerdings bleibt eine signifikante Diskrepanz zwischen Evidenz und Praxis im Bereich der akuten Schmerztherapie - diese Lücke zu schließen ist die größte Herausforderung.
