Echokardiografie bei akuter Herzinsuffizienz: von Ersteinschätzung in der Notaufnahme bis Therapieoptimierung auf der Intensivstation

Abstract
Echocardiography is a powerful but underused tool for the bedside assessment of patients with acute heart failure (AHF). The CME article summarizes recommendations for the use of echocardiography in AHF patients in the emergency department. In addition, it describes a step-by-step approach for the hemodynamic assessment of AHF patients in intensive care units.

Die bettseitige Echokardiografie ist von großem Nutzen bei Patienten mit akuter Herzinsuffizienz.

Aufgrund fehlender Kontraindikationen sollte die Indikation zur Echokardiografie großzügig gestellt werden.

Im Rahmen der Erstdiagnostik können echokardiografisch viele Ursachen einer akuten Herzinsuffizienz – wie akutes koronares Syndrom, dekompensierte Vitien, Lungenarterienembolie und Perikardtamponade – differenzialdiagnostisch abgegrenzt werden.

Nach primär gestellter Diagnose ermöglichen echokardiografische Verlaufsbeurteilungen auf der Intensivstation eine weitere Therapieoptimierung durch Beurteilung der aktuellen Kreislaufsituation.
