Zusammenfassung

Doping hat viele Facetten und beinhaltet u. a. den Einsatz von Stimulanzien, Narkotika, über Blut- und Peptidprodukte bis hin zu Hormonen und Anabolika. Das Internationale Olympische Komitee (IOC) definiert erstmals 1968 das Doping über eine Liste verbotener Wirkstoffe und Methoden.

Ähnlich einem Medikamentenmissbrauch kann das Doping im privaten Gebrauch bei Sportlern verstanden werden; unerlaubte Substanzen in unerlaubter Anwendung werden für eine Leistungssteigerung eingesetzt und stellen ein Risiko für eine Gesundheitsschädigung dar.

Bei einem 36-jährigen Bodybuilder traten innerhalb von 2 Wochen nach intrakutaner Injektion des Anabolikums Stanozolol in die rechte Schulter multiple Pusteln und Papulopusteln mit massiver, lokaler Entzündungsreaktion auf. Bei ausgeprägter Schmerzsymptomatik wurde zunächst Ibuprofen eingenommen. Eine orale Antibiose mit einem Cefalosporin der zweiten Generation führte nur zu kurzzeitiger Besserung der Symptomatik und lokal entzündlichen Reaktion. Mit Verdacht auf einen Herpes Zoster war extern eine Therapie mit Aciclovir ergänzt worden. Bei stationärer Aufnahme fanden sich in mikrobiologischen Abstrichen Keime der physiologischen Hautflora und Moraxella catarrhalis. Bei klinischem Bild einer superinfizierten Steroidakne leiteten wir unter flankierender Antibiose eine Therapie mit Aknenormin 40 mg 1-0-0 ein; darunter kam es zu einer Besserung des Hautbefundes.

Abstract

Doping in sports is a diverse problem and can be defined as the abuse of stimulant drugs, narcotics, hormones and anabolic substances. In 1968 a first list of prohibited drugs was declared by the IOC (International Olympic Committee). Doping in non-professional sports has many similarities to drug abuse and can induce severe health problems.

A 36 year bodybuilder was admitted to hospital two weeks after injection of stanozolol into the right shoulder. Initial treatment consisting of Ibuprofen and an oral antibiosis was started some days earlier by a GP. Our first working hypothesis was “steroid acne” and the patient was given doxycycline i. v. and Aknenormin 40 mg per os. Microbiological growth of Moraxella catarrhalis and physiologic flora of the skin was detected. Soon the patient recovered and could be discharged from hospital in good condition.
