Der Shunt ist die Lebensader der Dialysepatienten und es sollte selbstverständlich sein, sein Potenzial auszuschöpfen, um ihn langfristig zu erhalten. Allerdings haben wir festgestellt, dass entgegen dem aktuellen Wissensstand in vielen Dialyseeinrichtungen oftmals unnötige Arealpunktionen durchgeführt werden. Fast jeder kann die Arealpunktion problemlos durchführen. Sie verspricht eine oftmals schmerzarme oder sogar schmerzfreie Punktion und wird nach unserer Erfahrung ohnehin sehr häufig angewendet. Weshalb ist sie dennoch verpönt? Wir sind dieser Frage auf den Grund gegangen und haben überprüft, ob unsere Behauptung der Realität entspricht. Hierfür führten wir sowohl eine Bestandsaufnahme bzgl. des aktuellen Zustands der Shunts als auch Befragungen der Mitarbeiter und betroffenen Patienten in 5 Dialysezentren bzw. Dialysestationen in Krankenhäusern durch. In unserer Projektarbeit, auf der dieser Beitrag basiert, zeigen wir das Spektrum der Shuntpunktionstypen und Möglichkeiten einer optimalen Shuntpunktion auf. Wenn unsere Kollegen durch eine Schulung wieder bzgl. der Strickleiterpunktion sensibilisiert werden und dadurch Shuntkomplikationen zukünftig weitestgehend vermieden werden können, dann haben wir unser Ziel erreicht.
The shunt is the lifeline of dialysis patients and it should be self-evident to utilize its potential in order to preserve it as long as possible. But we had to consider that – contrary to current knowledge – there were often unnecessary areal punctures in the examined dialysis facilities. Nearly everybody can apply areal puncture without difficulty. It promises an often painless puncture and anyway is used very often in our opinion. Why do people nevertheless frown upon it? We got to the bottom of this question and checked if our assertion matches reality. We conducted a survey of the current state of the shunts as well as an interrogation of the staff and affected patients in 5 dialysis centers and stations in hospitals, respectively. In our project work, we show the spectrum of shunt puncture types and possibilities of optimal shunt puncture. If our colleagues are resensitized regarding rope ladder puncture via training and shunt complications are thereby reduced as far as possible in the future, we will have reached our goal.
