Der komplizierte Alkoholentzug: Grand-Mal-Anfälle, Delir und Wernicke-Enzephalopathie

Das Delirium tremens ist eine der häufigen Komplikationen eines Alkoholentzugs. Es kann potenziell tödlich enden und muss deshalb früh erkannt und intensiv überwacht und behandelt werden. Weil die differenzialdiagnostisch zu erwägende Wernicke-Enzephalopathie klinisch nicht sicher vom Delir abgegrenzt werden kann, sind hoch dosierte parenterale Vitamin-B1- sowie orale Magnesiumgaben spätestens bei einem schweren Entzug anzuraten. Leitliniengerecht wird ein Alkoholentzugsdelir mit Benzodiazepinen und ggf. mit Haloperidol bei Flüssigkeits- und Elektrolytsubstitution behandelt. Das Alkoholentzugsdelir klingt meist binnen 10 Tagen ab.
