Nutzung des Headgear-Effektes der M.A.R.A.-Apparatur (Mandibular Anterior Repositioning Appliance) in der Klasse II/2-Behandlung – ein Fallbericht

Zusammenfassung

In dem vorliegenden Patientenfall einer 17-Jährigen Patientin mit einer dentalen Klasse II/2 in Kombination mit einem deutlichen Tiefbiss konnte die gezielte Wirkungssteuerung der M.A.R.A.-Apparatur gezeigt werden.

Die reziprok wirkende Kraft auf die Oberkieferdentition (Headgear-Effekt) wurde durch die Extraktion der 2. Molaren begünstigt. Nach einer 5-monatigen Nivellierungsphase mit Brackets wurde das M.A.R.A. zur Bisslageeinstellung in eine Regelverzahnung für 8 Monate eingesetzt. Die aktive kieferorthopädische Behandlung war nach 18 Monaten mit Einstellung in eine stabile Klasse I bei physiologischem Overjet und Overbite abgeschlossen. Auf eine kurze Retentionsphase mit Duranschienen folgte die weitere Stabilisierung mit festsitzenden Retainern im Ober- und Unterkiefer. Auch 24 Monate nach Behandlungsende war das Ergebnis stabil.

Die M.A.R.A.-Apparatur erweist sich aus unserer über 10-jährigen Erfahrung damit als ein Fixed Functional Gerät, das sich sehr gut den individuellen, behandlerischen Bedürfnissen und Gewohnheiten in der Klasse-II-Behandlung bzgl. der Auswahl des Bracketsystems und der Bogensequenz anpasst. Darüber hinaus erfüllt die Apparatur die Ansprüche nach einem einfachen klinischen Handling, vorhersehbaren Behandlungsergebnissen bei vielfältigem klinischen Einsatzbereich bei Klasse-II-Anomalien bei Kindern, Jugendlichen und auch Erwachsenen. Eine hohe Patientenakzeptanz hinsichtlich Komfort und Ästhetik kann bestätigt werden.

Abstract

In this case report of a 17-year-old patient with a dental class II/2 in combination with a clear overbite, the targeted effect of the M.A.R.A. appliance could be shown.

The reciprocal force on the upper jaw dentition (headgear effect) was favoured by the extraction of the 2nd molars. After a 5-month levelling phase with brackets, the M.A.R.A. appliance was inserted for 8 months to adjust the bite position. Active orthodontic treatment was completed after 18 months with an adjustment to a stable Class I with physiological overjet and overbite. After a short retention phase with Duran splints, further stabilization with fixed retainers in the upper and lower jaws followed. The result remained stable for 24 months after the end of treatment.

Based on our more than 10 years of experience, the M.A.R.A. appliance therefore proves itself as a fixed functional device which can be adapted very well to individual treatment needs and practices in Class II treatment regarding the choice of the bracket system and archwire sequence. In addition, the appliance fulfils the requirements of simple clinical handling, foreseeable treatment results with a varied clinical area of application with Class II anomalies in children, youths and also adults.
