Lebensbedrohliche perioperative Komplikationen bei Ehlers-Danlos Syndrom mit Dissektion der Arteria carotis interna

Zusammenfassung.

Ziel der Studie: Die genauen Wirkmechanismen von Anästhetika an Strukturen des zentralen Nervensystems sind bis heute ungeklärt. Eine Klasse von Rezeptoren, deren Agonisten sich durch eine ausgeprägte sedierende Wirkung auszeichnen sind die α₂-Adrenozeptoren. Neben den Wirkungen der klassischen Agonisten sind in jüngerer Zeit auch Effekte von klinisch verwendeten Anästhetika, z. B. Etomidat, auf α₂-Adrenozeptoren beschrieben worden. Ziel dieser Studie war daher die Untersuchung des Effekts anderer intravenöser Anästhetika, wie Propofol und Ketamin, auf die Bindung des spezifischen α₂-Adrenozeptoragonisten Parajodoclonidin an cerebrale α₂-Adrenozeptoren. Methodik: Nach Zustimmung der örtlichen Tierschutzkommission wurde das Hirngewebe von sechs Wistar-Ratten bei -80 °C eingefroren. Danach wurden Schnitte von 15 - 20 µm Dicke angefertigt. Nach Waschen der Schnitte in TRIS-Puffer (pH = 7,5) wurden die Hirnschnitte mit 2,5 nM ¹²⁵J-Parajodoclonidin (PIC) inkubiert, um die spezifische Bindung zu bestimmen. Die unspezifische Bindung wurde nach Zugabe von 200 µM nicht markiertem Clonidin gemessen. Die Verdrängung von PIC von zerebralen α₂-Adrenozeptoren wurde dann in Gegenwart von Propofol (11 - 530 µM) sowie Ketamin (52 - 1100 µM) autoradiographisch untersucht. Dazu wurden die Schnitte für drei Tage auf einen sensitiven Film aufgebracht. Die Auswertung erfolgte densitometrisch. Statistische Signifikanz wurde mit dem Student t-Test untersucht. Resultate: Nur in den höchsten untersuchten Konzentrationen von Propofol und Ketamin konnte ein Effekt der Substanzen auf die Bindung von PIC nachgewiesen werden. Weder Propofol noch Ketamin waren in der Lage PIC komplett von zerebralen α₂-Adrenozeptoren zu verdrängen. Diskussion: Im Gegensatz zu anderen Anästhetika wie zum Beispiel Etomidat, für das eine komplette Verdrängung von PIC aus cerebralen Schnitten demonstriert werden konnte, zeigten die intravenösen Anästhetika Propofol und Ketamin nur einen geringen Effekt auf die Bindung von PIC an α₂-Adrenozeptoren. Ein weiterer Wirkort dieser Substanzen an zerebralen α₂-Adrenozeptoren neben den beschriebenen Effekten auf GABA-Rezeptoren bzw. NMDA-Rezeptoren erscheint daher unwahrscheinlich.

Objective: Up to date the exact mechanisms of action of anaesthetics on structures of the central nervous system have not been elucidated. Agents acting on central α₂-adrenoceptors have been demonstrated to possess potent sedative properties. Beside the classical agonists, general anaesthetics that are currently in clinical use e.g. etomidate have also been shown to act on α₂-adrenoceptors. Thus, the aim of this study was to study the effect of the intravenous anaesthetic agents propofol and ketamine on the binding of the specific α₂-adrenoceptor agonist paraiodoclonidine at cerebral α₂ adrenoceptors. Methods: After approval of the local animal care committee brain tissue of six Wistar rats was removed and frozen at -80 °C. The tissue was sectioned in 15 - 20 µm thick slices. After washing in TRIS-buffer (pH = 7.5) the slices were incubated with 2.5 nM ¹²⁵I-paraiodoclonidine (PIC) for determination of specific binding. Unspecific binding was determined in presence of 200 µM unlabeled clonidine. Displacement experiments were performed in presence of propofol (11 - 530 µM) and ketamine (52 - 1100 µM) and evaluated autoradiographically. Average exposure time was three days. All films were then analysed by densitometry. Statistical significance calculated by Student's t-test was assumed at a level of p < 0.05. Results: Only at the highest concentration of propofol and ketamine an effect on PIC binding was noticeable. Neither propofol nor ketamine was able to displace PIC completely from cerebral α₂-adrenoceptors. Conclusion: In contrast to other anaesthetics such as etomidate which was demonstrated to displace PIC completely from α₂-adrenoceptors, propofol and ketamine exerted only minor effects on the binding of PIC even at concentrations that exceeded clinical concentrations. These data suggest that an additional site of action of these anaesthetics at cerebral α₂-adrenoceptors beside the known actions on GABA receptors or NMDA receptors is unlikely.
