Ambulante Diagnostik und Therapie der Gonarthrose

Zusammenfassung.

Fragestellung: Ziel der vorliegenden Arbeit ist die Zusammenstellung von Empfehlungen zur ambulanten Diagnostik und Therapie der Gonarthrose. Methodik: Im Rahmen eines Expertengespräches wurde in einem nominalen Gruppenprozeß ein Textvorschlag erarbeitet, der im Anschluß von allen Beteiligten konsensfähig gemacht wurde. Ergebnisse: Eine standardisierte Durchführung der klinischen Untersuchung und der bildgebenden Diagnostik ist vor der Einleitung therapeutischer Maßnahmen notwendig. Neben einer grundsätzlich indizierten röntgenologischen Darstellung des Kniegelenkes in zwei senkrecht aufeinanderstehenden Ebenen (a.p. und seitlicher Strahlengang) sowie eventuell zusätzlicher Tangentialaufnahme der Patella ist eine Kernspintomographie nur indiziert, wenn eine differentialdiagnostische Klärung zusätzliche bildgebende Maßnahmen erfordert. Physikalische Therapiemaßnahmen, Hilfsmittel und medikamentöse Substanzen sind in Abhängigkeit von den jeweiligen Therapiezielen bzw. Erkrankungsstadien indiziert. Intraartikuläre Injektionen und Punktionen sollten unter Berücksichtigung publizierter Leitlinien vorgenommen werden. Schlußfolgerungen: Um unter gesundheitsökonomischen Gesichtspunkten eine langfristig sinnvolle Behandlung der Gonarthrose zu gewährleisten, empfehlen die Autoren einen rationalen Einsatz der zusammengestellten diagnostischen und therapeutischen Maßnahmen.

Aim of study: To develop recommendations for appropriate diagnostic procedures and conservative treatment of knee osteoarthritis in outpatients. Material and methods: Following a consensus conference and expert reviews basic recommendations were developed. Results: While standardized radiographic assessment is mandatory, MRI investigation should be restricted to problems apart from osteoarthritis. Indications for physical therapy, bracing and pharmacotreatment depend on the severity of the disease. Guidelines for intraarticular injections are presented. Conclusion: Effective treatment of knee osteoarthritis must be based on available recommendations and guidelines.
