A Fibreoptic Scoring System to Assess the Position of Laryngeal Mask Airway Devices. Interobserver Variability and a Comparison Between the Standard, Flexible and Intubating Laryngeal Mask Airways

Summary.

We determined the interobserver reliability of a fibreoptic scoring system for assessing the position of the laryngeal mask airway (LMA), the flexible laryngeal mask airway (FLMA) and the intubating laryngeal mask airway (ILM). We also compare fibreoptic position between the devices. Thirty anaesthetised adult patients were studied in random order in a triple crossover manner. Two observers blinded to each others findings scored the fibreoptic position as follows: 4, only vocal cords visible; 3, vocal cords plus posterior epiglottis visible; 2, vocal cords plus anterior epiglottis visible; 1, vocal cords not seen. Interobserver reliability was examined using intraclass correlation coefficient (ICC). Interobserver reliability was graded as excellent for the LMA (ICC = 0.89), FLMA (ICC = 0.87) and ILM (ICC = 0.79). Fibreoptic scores were higher for the LMA and FLMA compared with the ILM (both p<0.001). We conclude that interobserver reliability for the fibreoptic scoring system is excellent for the LMA, FLMA and ILM. Fibreoptic position is better for the LMA and FLMA compared with the ILM. Fibreoptic scoring has potential utility for research and clinical practice with laryngeal mask devices.

Ein fiberoptisches Score-System zur Beurteilung der Position von Larynxmasken. Interindividuelle Untersuchervariabilität und ein Vergleich zwischen Standard-, Flexibler und Intubationslarynxmaske.

Wir bestimmten die interindividuelle Verlässlichkeit eines fiberoptischen Score Systems für die Beurteilung von Standard-Larynxmasken (LMA), flexiblen Larynxmasken (FLMA) und Intubationslarynxmasken (ILM). Wir vergleichen auch die fiberoptische Positionierung zwischen den verschiedenen Masken. Dreißig anästhesierte erwachsene Patienten wurden nach dem Zufallsprinzip in einem triple cross-over untersucht. Zwei Untersucher, die in ihren Ergebnissen gegeneinander blind waren, bewerteten die fiberoptische Position wie folgt: 4, nur Stimmbänder sichtbar; 3, Stimmbänder plus hintere Epiglottis sichtbar; 2, Stimmbänder plus vordere Epiglottis sichtbar; 1, Stimmbänder nicht sichtbar. Die interindividuelle Verlässlichkeit wurde nach dem Intraclass Korrelationskoeffizient (ICC) beurteilt. Die interindividuelle Verlässlichkeit wurde bei der LMA als hervorragend eingestuft (ICC = 0,89). FLMA (ICC = 0,87) und ILM (ICC = 0,79). Die fiberoptischen Scores waren bei der LMA und FLMA höher als bei der ILM (beide p < 0,001). Wir schließen aus diesen Ergebnissen, dass die interindividuelle Verlässlichkeit für das fiberoptische Score-System für LMA, FLMA und ILM als hervorragend eingestuft werden kann. Die fiberoptische Position ist bei LMA und FLMA besser als bei der ILM. Das fiberoptische Score-System ist von potenziellem Nutzen für Untersuchungen und den klinischen Alltag mit Larynxmasken.
