Ultraschall-Elastographie der Prostata

Zusammenfassung

Studienziel: Tumoren der Prostata können eine größere mechanische Härte aufweisen als das sie umgebende Gewebe. Deshalb sind bei der transrektalen digitalen Tastbefundung nicht nur Vergrößerungen des Organs, sondern auch lokale Verhärtungen feststellbar. Diese Untersuchungstechnik ist z. Z. das Verfahren mit der größten Spezifität, ist aber selbst in Kombination mit der Auswertung von PSA-Werten und der Untersuchung mit transrektalem Ultraschall in seiner Aussage nicht sicher. Das Verfahren der ultraschallgestützten Elastographie erlaubt hingegen die Erfassung und Visualisierung mechanisch verhärteter Gewebebereiche mit höherer Empfindlichkeit und Genauigkeit als mit dem Finger und stellt somit eine nützliche Ergänzung zu herkömmlichen Untersuchungsmethoden dar. Methode: Wir haben das Verfahren der Ultraschall-Elastographie für die Prostatadiagnostik entwickelt. Bei der transrektalen Untersuchung werden Ultraschallbildfolgen aufgenommen, während das Organ mit der manuell geführten Ultraschallsonde geringfügig komprimiert wird. Durch numerische Auswertung der bei verschiedenen Kompressionsstufen automatisch aufgenommenen Einzelbilder können Dehnungs- bzw. Kompressionsbilder berechnet werden, die die räumliche Verteilung der elastischen Gewebeeigenschaften in bestimmten Organquerschnitten repräsentieren. Mit diesem Verfahren können insbesondere lokale Verhärtungen sichtbar gemacht werden. Verfälschungen, die durch seitliches Ausweichen von Gewebebereichen, d. h. orthogonal zum Kompressionsdruck, entstehen können, werden bei dem Verfahren durch einen speziell entwickelten Ansatz kompensiert. Ergebnisse: Wir präsentieren Aufnahmen an einem Elastographiephantom und In-vivo-Ergebnisse von Patienten vor einer radikalen Prostatektomie, die belegen, dass die Ultraschall-Elastographie das Potenzial besitzt, krankes Tumorgewebe zu detektieren, das im herkömmlichen B-Bild unauffällig ist. Der Befund ist mit Hilfe der korrespondierenden histologischen Präparate nachgewiesen.

Aim: Prostate tumours are often of harder consistency than the surrounding tissue. During digital rectal examination, this fact can be used not only to detect hypertrophy but also localized hardenings. The examination by digital palpation is inaccurate and, even in combination with PSA-value and a transrectal ultrasonic examination, the result is often not reliable. Ultrasound elastography enables us to measure and visualize the elastic properties of a tissue region and is a useful supplement to the examination by digital palpation. Method: We have developed a new system for elastographic prostate diagnosis which can be used during the transrectal ultrasonic examination. During the examination a sequence of ultrasonic images is acquired while the organ is slightly compressed by the ultrasound probe. Using numerical analysis of image pairs for the acquired sequence we calculate the tissue strain which represents the spatial elasticity distribution of a specific cross-section of the organ. This enables us to distinguish hard areas in the tissue. Image artifacts resulting from lateral motion components, i.e., orthogonal to the direction of the applied force, are compensated for by a special approach. Results: We present results obtained from a typical elastography phantom and also the first in vivo images from patients who were undergoing radical prostatectomy. Our images prove that ultrasound elastography has the potential to detect malignant tissue areas, which are inconspicuous in the B-mode image. Our findings are confirmed by the corresponding histological specimens.
