Spontanruptur einer Magenwandmetastase als Erstmanifestation eines Nierenzellkarzinoms. Kasuistik und Literaturübersicht

Zusammenfassung

Einleitung: Fernmetastasen im oberen Gastrointestinaltrakt bei Patienten mit Nierenzellkarzinom sind sehr selten und wurden bisher meist im Rahmen von Autopsiestudien entdeckt. Kommt es zu Beschwerden von Seiten dieser Fernmetastasen, so steht i. d. R. die obere gastrointestinale Blutung mit Hämatemesis, Melaena und Anämie im Vordergrund der akuten klinischen Symptomatik.Kasuistik: Bei einem 56-jährigen Mann wurde aufgrund einer unklaren intraabdominellen Blutung eine Laparotomie durchgeführt, bei der ein rupturierter Tumor an der Magenwand als Blutungsquelle identifiziert werden konnte. Histologisch entsprach das Tumorexzidat aus der Magenwand am ehesten der Metastase eines Nierenzellkarzinoms. Im Rahmen der weiteren Diagnostik wurde ein bislang unbekannter Nierentumor entdeckt und der Patient im Intervall tumornephrektomiert. Ein histologischer Vergleich des Nierenzellkarzinoms der linken Niere mit dem Exzidat aus der Magenwand ergab gleichartiges Gewebe.Die Erstmanifestation eines Nierenzellkarzinoms durch eine intraabdominelle Blutung bei rupturierter Magenwandmetastase ist bisher in der Literatur nicht beschrieben.Schlussfolgerung: Angesichts der in der Literatur beschriebenen Kasuistiken sollte bei Patienten mit Nierenzellkarzinomen an die Möglichkeit einer gastrointestinalen Filialisierung gedacht und bei klinischem Hinweis eine weitere (endoskopische) Abklärung angestrebt werden.

Abstract

Introduction: The incidence of tumor metastasis to the upper GI-region in patients with renal call carcinoma is very uncommon. Gastric metastatic tumors in these patients have been mainly reported in autopsy series. In general hematemesis, melena and anemia according to upper GI bleeding are the main clinical problems in patients with tumor metastasis to the stomach.Case report: A 56 year old man presented to the emergency room with an intraabdominal bleeding of unknown origin. The patient was explored transperitoneally. A ruptured tumor of the gastric wall could be identified as the source of bleeding. Histologic examination of the resected gastric tumor showed morphologic appearance consistent with the metastasis of a renal cell carcinoma. Further diagnostic means revealed a tumor of the left kidney and the patient underwent radical nephrectomy 15 days after laparotomy. The histomorphologic examination of the renal tumor showed a renal cell carcinom that was identical with the histology of the resected gastric mass.We didn't find other cases in the literature that described a spontaneous rupture of a metastasis to the gastric wall as initial manifestation of a renal cell carcinoma.Conclusions: Metastatic disease involving the stomach in patients with renal carcinoma is very rare. But a gastrointestinal metastasis should be thought of in these patients and if there is clinical evidence a gastrointestinal endoscopy should be performed.
