Lebensqualitätsforschung in der Urologie - lohnt die Mühe?

Zusammenfassung

Medizinische Behandlung zielt in der Regel zunächst auf die Sicherung des Überlebens, dann auf die Vermeidung künftiger Gesundheitsschäden und erst danach auf die Lebensqualität. Die verbesserte Effektivität und Vielfalt moderner medizinischer Behandlungsmöglichkeiten jedoch lässt die Lebensqualität für das medizinische Handeln zunehmend bedeutsam werden. Da urologische Erkrankungen häufig die Lebensqualität der Patienten stark beeinträchtigen, ist gerade in diesem Bereich Lebensqualitätsforschung besonders wichtig. Andererseits sind aber wesentliche Beiträge zur Lebensqualitätsforschung in der Urologie schon geleistet worden. Diese Übersicht zeigt dies an drei Beispielen: Nierentransplantation, Prostatakarzinom und Harnableitung. In jedem der drei Bereiche zeigen sich spezifische Einschränkungen in der Lebensqualität, die mit der Art der Behandlung und Komorbiditäten variieren.

Abstract

In general, the primary goal of medical treatment is the assurance of survival, followed by the avoidance of future health problems, and only thereafter, the quality of life. Nevertheless, the improved effectiveness and the diverse possibilities of modern medical treatment have let quality of life become increasingly significant for the medical business. Because urologic diseases are often detrimental for a patient's quality of life, it is an especially important area of research for quality of life. On the other hand, substantial contributions have already been made in quality of life research in the field of urology. In the following overview, this will be shown with three examples: kidney transplant, prostate carcinoma, and urinary diversion. In each of the three areas, specific restrictions in quality of life are shown which vary with the type of treatment and co-morbidities.

2 Rechnerisch lässt sich dies beispielsweise so durchführen: Man bestimmt die Differenz einer Patientengruppe zur Referenzpopulation aus der Normalbevölkerung in Form von Mittelwertsabweichungen geteilt durch die Standardabweichung und erhält so einfach zu interpretierende z-Werte, wie sie weiter unten berichtet werden. Unter Annahme einer annähernd normalverteilten Skala bedeutet ein z-Wert von - 1 beispielsweise, dass ca. 15 % der Normalbevölkerung gleiche oder niedrigere Werte aufweisen, die übrigen 85 % höhere Werte. Die prozentualen Anteile zu anderen z-Werten sind in Tabellen enthalten.
