Zusammenfassung
Fragestellung: Ureteroskopie (URS) und extrakorporale Stoßwellenlithotripsie (ESWL) sind nach wie vor konkurrierende Behandlungsmodalitäten beim Harnleiterstein. Aufgrund der „Invasivität” der Methode, der Notwendigkeit für eine allgemeine Anästhesie und der häufig postoperativ erforderlichen Einlage eines Harnleiterstents wird die URS oft als Methode der zweiten Wahl betrachtet.Ziel unserer Untersuchung ist es, anhand des Patientenkollektivs unserer Klinik die Effizienz der URS bezüglich der primären Steinfreiheit und der generellen Erfolgsquote zu beurteilen, Komplikationen zu quantifizieren und die Ursachen für erfolglose Eingriffe zu analysieren.Material und Methode: Die ureteroskopischen Eingriffe in unserer Klinik aus der Zeit vom 1. 7. 96 bis zum 30. 4. 99 wurden retrospektiv ausgewertet. Es wurden 215 Eingriffe bei 196 Patienten (66 Frauen, 130 Männer) durchgeführt. Es erfolgte eine Auswertung bezüglich des Zeitpunktes und der Quote der Steinfreiheit, der Anwendung auxiliärer Maßnahmen und der Komplikationen.Ergebnisse: Die URS war in 208/215 Fällen durchführbar, die kumulierte Steinfreiheit betrug 94,6 %. Primäre Steinfreiheit wurde bei 114 Patienten erzielt. Primäre Steinfreiheit wird häufiger im distalen Ureter (70,8 %) als im mittleren Ureter (55,8 %) oder im oberen Ureter (21,3 %) erzielt, Patienten mit kleinen Steinen (< 5 mm) sind häufiger primär steinfrei (71,5 %) als Patienten mit größeren Steinen (> 5 mm, 28,2 %). Die OP-Zeit betrug durchschnittlich 36 min. Komplikationen sind mit 13,7 % selten, schwere operationsbedingte Komplikationen wie Verlust einer Niere oder Harnleiterabriss traten bei 2 Patienten auf.Schlussfolgerung: Die URS ist eine komplikationsarme Methode zur Behandlung des Harnleitersteines mit hoher Effizienz. Sie kann im unteren Drittel als Methode der ersten Wahl gelten und ist im mittleren Drittel ein der ESWL gleichwertiges Verfahren. Im oberen Drittel kann die Indikation bei ohnehin notwendiger retrograder Intervention großzügig gestellt werden.Insbesondere für die Behandlung der Steine im mittleren und oberen Drittel sollte das Konzept individuell erstellt werden.
Abstract
Purpose: Ureteroscopy (URS) and extracorporeal shock wave lithotripsy are competing means in the treatment of ureteral calculi. Invasiveness, need for general anaesthesia and routine stenting are the major disadvantages of URS. Therefore, URS is still thought to be a method of second choice.We report our experience with URS as „first-line” method in the treatment of ureteral stones.Material and methods: From July 1st 1996 to April 30rd a total no of 215 ureteroscopic interventions for ureteral calculi was performed in 196 patients (66 female, 130 male). Primary and overall stonefree rate was calculated. Operating time, use of ESWL and other auxillary means were analysed as well as complications and unsuccessfull operations.Results: We are able to perform URS in 208 out of 215 cases. The overall stonefree rate was 94.6 %. 114 patients were primarily stonefree. This was the case in 70.8 % of patients with the stone in the distal ureter, in 55.8 % in the mid ureter and in 21.3 % in the upper ureter. In patients with smaller stones (< 5 mm) 71.5 % were primarily stonefree compared to 28.2 % in patients with larger stones (> 5 mm).Mean operating time was 32 min (15 - 55 min). Complications were rare with a rate of 13.7 %. Severe complications like loss of kidney or disruption of the ureter occurred in 2 patients.Conclusions: URS is a highly efficient method to clear ureteral stones with little complications. It is the method of choice in distal ureteral calculi. In the mid ureter URS seems to be equiefficient with ESWL. If endoscopic intervention is requiered for stones in the upper ureter (e. g. because of stone burden or persistent colic pains) immediate URS should be considered.An individualized treatment-concept for ureteral stones especially in the mid and upper ureter is useful.
