ABSTRACT
Unhealed perineal wounds are a difficult and challenging problem for the colorectal surgeon. They occur most frequently after extensive abdominoperineal wounds subjected to radiotherapy and after proctectomies for inflammatory bowel disease. Etiologic factors and the pathophysiology of these wounds are reviewed. Treatment options ranging from wound care to flap closures are presented along with newer adjuvant therapies.
