Morphologische und biochemische Veränderungen bei der Dupuytren'schen Kontraktur

Zusammenfassung

Die Diagnose der Dupuytren'schen Kontraktur basiert auf klinischen Angaben und Funktionsprüfungen. Es wird nur vereinzelt über Laboruntersuchungen berichtet. Im Blut von Patienten mit Dupuytren'scher Erkrankung wurden normale Konzentrationen von PMN-Elastase, α1-Antitrypsin, α2-Makroglobulin, Interleukin-1β, Fibronektin, β-D-Xylosyltransferase, Prokollagen-III-Peptid und von den einzelnen Glykosaminoglykanen gemessen. Jedoch hatten die Patienten mit Dupuytren'scher Erkrankung signifikant niedrigere Kollagenase- und Gelatinase-Konzentrationen gegenüber der gesunden Referenzpopulation. Die Konzentration von Glykosaminoglykan-Disacchariden in Patienten mit Dupuytren'scher Kontraktur unterscheiden sich signifikant gegenüber Gesunden. Die Disaccharide von Chondroitin und von Chondroitin-4-sulfat sind auf fast das Doppelte gegenüber den entsprechenden Werten im Serum Gesunder erhöht (p = 0,001). Die Hyaluronan-Disaccharide im Patientenserum sind sogar auf das 7-fache und signifikant (p = 0,001) gegenüber den entsprechenden Werten im Serum von gesunden Probanden erhöht. Die Disaccharide von Dermatan-4-sulfat sind bei den pathologischen Seren nicht nachweisbar (p = 0,001).

Abstract

The diagnosis of Dupuytren's disease is based on clinical rather than serological parameters. We investigated blood samples collected from patients with manifest Dupuytren's contracture. Normal concentrations of PMN elastase, α1-antitrypsin, α2-macroglobulin, interleukin-1β, fibronectin, β-D-xylosyltransferase, propeptide of type III collagen and single glycosaminoglycans were found. Only the concentrations of collagenase and gelatinase were found significantly lower compared to the mean values in healthy subjects. The concentrations of glycosaminoglycan disaccharides in patients with Dupuytren's contracture are significant different compared to a healthy reference population. The chondroitin and chondroitin-4-sulphate disaccharides are significant increased, twice as much. The concentration of hyaluronan disaccharides was 7-fold higher than in controls (p = 0,001). In contrast the disaccharides of dematan-4-sulphate were decreased significantly.
