Zusammenfassung.
Die Verletzung der Pleura ist eine häufige Komplikation der Punktion zentraler Venen. Besonders während Überdruckbeatmung kann sich wie in dem vorliegenden Fall der Implantation eines Automatischen Implantierbaren Cardioverter Defibrillator (AICD) ein Spannungspneumothorax entwickeln. In der Regel ist dieser von charakteristischen Symptomen wie erhöhter Beatmungsdruck, Hypoxämie, ansteigendem zentralen Venendruck, Hypotonie und Tachykardie gekennzeichnet. In diesem Fall blieb jedoch ein ausgeprägter Spannungspneumothorax ohne klinische Symptomatik.
Tension Pneumothorax After Subclavian Vein Puncture for ICD-Implantation Without Cardiopulmonary Effects.
Injury of the pleura is a common complication of central vein puncture. It may occur especially under positive pressure ventilation as in a 4 hours lasting implantation of an Automatic Implantable Cardioverter Defibrillator Device (AICD) as a tension pneumothorax, which is usually accompanied by of typical clinical symptoms. None of the typical signs as increased ventilatory pressure, hypoxaemia and cardiac supression, which lead to rising central venous pressure, hypotension and tachycardia, were observed.
