Zusammenfassung
Die zentrale pontine Myelinolyse stellt eine schwerwiegende Komplikation bei der Korrektur einer Hyponatriämie dar. Therapieempfehlungen jüngerer Zeit betonen die Wichtigkeit einer langsamen Elektrolytkorrektur. Wir berichten über eine 26-jährige alkoholkranke und unterernährte Patientin, bei der eine schwerwiegende zentrale pontine Myelinolyse trotz langsamer Elektrolytkorrektur einer ausgeprägten Hyponatriämie (Serum-Natrium initial 97 mmol/l) auftrat. Die neurologischen Ausfälle bildeten sich über 2œ Monate weitgehend zurück. Die Ursachen der zentralen pontinen Myelinolyse und die aktuellen Therapieempfehlungen werden diskutiert.
Central Pontine Myelinolysis in Spite of Slow Correction of a Severe Hyponatremia
Central pontine myelinolysis is a severe complication following the correction of hyponatremia. Current therapeutic recommendations emphasize the need of slow correction of the electrolyte imbalance. We report on a 26-year old alcohol-dependant woman, who developed central pontine myelinolysis in spite of slow correction of a severe hyponatremia (serumsodium 97 mmol/l). Her severe neurological impairment improved over 2œ months nearly completely. We discuss the causes of central pontine myelinolysis and therapeutical recommendations.
