Anamnese

Ein 73-jähriger Patient stellte sich wegen seit 5 Wochen bestehender progredienter Festkörperdysphagie in unserer Klinik vor.

Der Patient gab einen Gewichtsverlust von 5 kg seit Beschwerdebeginn an.

Anamnestisch war ein Merkell-Zell-Karzinom mit Erstdiagnose vor 2 Jahren bekannt bei einer Primärlokalisation am rechten Unterschenkel mit ipsilateraler inguinaler Lymphknotenmetastase. Initial erfolgte eine Tumorresektion mit inguinaler Lymphknotendissektion und adjuvanter Radiatio.

4 Wochen vor der Erstvorstellung war eine progrediente kleinknotige, kutane Manifestation am rechten Unterschenkel sowie eine Lymphknotenmetastase rechts-inguinal nachgewiesen worden. In einer CT-Untersuchung des Abdomen zeigte sich eine große retroperitoneale metastasenverdächtige Raumforderung auf Höhe der linken Niere.
