Zusammenfassung
Engpasssyndrome stellen häufige Komplikationen rheumatischer Erkrankungen dar. Mit Abstand am häufigsten findet sich ein Karpaltunnelsyndrom. Daneben kann fast jeder periphere Nerv betroffen sein. Ursächlich kommen mechanische Kompression durch Schwellungen, Zysten, Gelenkfehlstellungen und Druck durch Orthesen in Frage. Ganz wesentlich ist es, hiervon die Mononeuritis multiplex, die eine, wenn auch seltene Komplikation zahlreicher rheumatischer Erkrankungen ist, abzutrennen. Die Behandlung muss in enger Kooperation zwischen Rheumatologen, Neurologen und Chirurgen erfolgen.
Entrapment Syndromes
Entrapment syndromes are common complications of rheumatic diseases. The carpal tunnel syndrome is the by far most common entrapment syndrome. Nearly all other peripheral nerves can be affected in few patients. Entrapment syndromes can be caused by swelling of the joints, cysts, deviation of the joints and external pressure by orthetic devices. A critical point is to distinguish entrapment neuropathies from mononeuritis multiplex which is a rare complication of many rheumatic disorders. Treatment must be managed by a team consisting of a rheumatologist, neurologist and a surgeon.
