Der intestinale Nabelschnurpolyp

Zusammenfassung

Wir berichten über die Morphologie und Pathogenese, über die Komplikationen und Differenzialdiagnose eines Dünndarmschleimhautpolypen an der Nabelschnur. Dieser fand sich postnatal an der Nabelschnur eines gesunden männlichen Reifgeborenen. Die Präsenz von Dünndarmschleimhaut auf der Oberfläche der Nabelschnur ist erklärbar durch die Persistenz von Anteilen des Ductus omphalomesentericus mit frühem Prolaps an die Nabelschnuroberfläche und Differenzierung sowie Proliferation der intestinalen Zellen.
Der Ductus omphalomesentericus ist eine tubuläre Struktur, die während der embryonalen Entwicklung den primitiven Darm mit dem Dottersack des Embryos verbindet. Normalerweise obliteriert und verschwindet dieser bis zur 10. Entwicklungswoche. Persistieren Anteile oder der gesamte Duktus, können sich differierende Anomalien in Form von Polypen, Fisteln und Zysten in und an der Nabelschnur sowie dem kindlichen Darm entwickeln, mit teils erheblichen klinischen Konsequenzen. Bei tumorösen Veränderungen an der Nabelschnur sind Myxome, Angiome sowie Teratome differenzialdiagnostisch abzugrenzen.

Abstract

The morphology, pathogenesis, complications and differential diagnosis of an intestinal polyp of the umbilical cord are presented. The polyp were detected postnatal on the umbilical cord in an healthy male newborn. The presens of intestinal tissue upon the umbilical cord ist possibel about the persistence from remnants of the ductus omphalomesentericus with prolaps and differentiation of the intestinal cells. The ductus omphalomesentericus is a tubular structure, a communication between the developing embryonic gut and the yolk sac, forming during the early embryonic life. Obliteration of the omphalomesenteric duct is usually complete by the 10^th week of gestation. Various portions of the duct may persist, however, giving rise to polyps, fistulas or cysts of the umbilical cord with potencially dangerous clinical consequences. Other tumors of the umbilical cord are myxoma, angioma and teratoma are differential diagnosis.
