Zusammenfassung
Material: Von 1/1998 bis 5/2000 wurden 1043 (100 %) Ersatzplastikoperationen prospektiv mit der Fragestellung nach Komplikationen bzw. Problemen erfasst. Das Durchschnittsalter betrug 32 Jahre (von 14 bis 62 Jahre), 371 waren weiblich, 672 männlich.
Ergebnisse: Intraoperative Komplikationen traten in 1,7 % der Fälle auf (IF-Schraube: 12, Blockfraktur: 3, Patellafraktur: 3). Es zeigten sich 2,4 % perioperative Komplikationen: 18 (1,7 %) Infektionen (16 tiefe/2 oberflächliche), bei 7 (0,7 %) Patienten fand sich ein revisionsbedürftiger Hämarthros/Hämatom. Postoperative Komplikationen (2.-6. Monat) gab es bei 8 (0,8 %) Patienten (7 × starkes Bewegungsdefizit, 1 × Patellafraktur). Bei 372 Patienten wurde die Metallentfernung durchgeführt. Hier erfolgte bei Beschwerden eine Second-look-Operation. 62 (16,7 %) gaben noch Schmerzen an (41 × Belastung, 14 × distaler Patellapol, 4 × diskret, 3 × bei endgradiger Flexion/Extension). 35 (9,4 %) zeigten eine Bewegungseinschränkung (26 endgradige und 9 größere Einschränkungen). Restliche Probleme 21 (5,6 %): Schnappgefühl, Erguss, Instabilitätsgefühl. 37 (9,9 %) Zyklopstumore, 19 (5,1 %) Fälle von Verwachsungen, 4 (1,1 %) × Transplantatlockerung und 2 × freie Körper fanden sich bei der Second-look-Op.
Schlussfolgerung: Die Ersatzoperation des vorderen Kreuzbandes mit mittlerem Patellarsehnendrittel in arthroskopischer Press-Fit-Technik ist ein anspruchvolles und bei regelmäßiger Anwendung komplikationsarmes Verfahren. Bei uns kam es in 4,9 % zu intra-, peri- bzw. postoperativen Komplikationen. Schwerwiegendere Probleme zum Zeitpunkt der Metallentfernung (Schmerzen distaler Patellapol, höhergradige Bewegungseinschränkung und Transplantatlockerung) zeigten sich in 7,2 % der Fälle.
Abstract
Material: From January 1998 to May 2000 we analyzed 1043 ACL-reconstructions concerning problems and complications. The average patient age was 32 years (from 14 to 62 years), 371 women and 672 men.
Results: We saw 1.7 % intra-operative complications (interference screw, fracture of the patella and fracture of the bone back). Furthermore in 1.7 % of the cases we had peri-operative complications including infections (16 deep and 2 superficial infections). Postoperative complications (2 to 6 month after operation) were found in 0.8 % including marked deficit in range of motion and fracture of the patella. In 372 patients a removal of the tibial fixation screw and a second look arthroscopy was performed if the patient had pain in the operated knee. 62 (16.7 %) patients reported pain (41 while walking, 14 at the distal pole of the patella, 4 mild pain, 3 in full extension/flexion). Deficit in range of motion was shown in 35 (9.4 %) patients (26 with mild deficit and 9 with marked deficit). Additional problems were seen in 21 (5.6 %) patients at this time: Feeling of snap, effusion, feeling of instability. In the second look arthroscopy we found 37 (9.9 %) cases of cyclops, 19 (5.1 %) patients showed adhesions and 4 a graft laxity. In 2 cases there were loose bodies.
Conclusion: The arthroscopic ACL-reconstruction in pressfit technique with the mid third of the patellar tendon is a demanding operation and if regularly done a procedure with little complications. We had 4.9 % intra-, peri- and postoperative complications. Marked problems at the time of the removal of the tibial fixation screw were encountered in 7.2 % of the cases including pain at the distal pole of the patella, marked deficit in range of motion and graft laxity.
