Zusammenfassung
Subarachnoidale Einblutungen nach Schädel-Hirn-Traumen können erst seit Einführung der Computertomographie sicher diagnostiziert werden. Die Angaben in der Literatur zur Häufigkeit und der prognostischen Bedeutung der traumatischen Subarachnoidalblutung (tSAB) sind widersprüchlich. Retrospektiv wurden 252 an der Universität Leipzig behandelte Schädel-Hirn-Traumatisierte untersucht. Bei 27,8 % der Patienten fand sich iminitialen Computertomogramm eine tSAB. Das Vorkommen der tSAB nimmt mit der Schwere des Schädel-Hirn-Traumas zu. Diese Blutungen als Zeichen der primären traumatischen Hirnschädigung korrelieren signifikant mit dem klinischen Ergebnis nach Schädel-Hirn-Trauma. Die traumatische Subarachnoidalblutung stellt deshalb einen wesentlichen diagnostischen Aspekt bei SHT-Patienten dar.
Abstract
In head injury, traumatic subarachnoid hemorrhage (t-SAH) is diagnosed using cranial computed tomography. In the literature, information concerning incidence and prognostic value of t-SAH are contradictory. 252 patients suffering from head injury were investigated retrospectively. Signs of t-SAH were found in 27.8 % of patients. The lower the initial Glasgow-coma-scale, the higher the incidence of t-SAH was found. The extend of t-SAH correlated significantly with the clinical outcome. Recognition of t-SAH is an important diagnostic, as well as prognostic aspect in the evaluation of head injury.
