Zusammenfassung
Obwohl das Hepatozelluläre Karzinom (HCC) eine der häufigsten Tumorerkrankungen weltweit ist, sind kurative Therapien limitiert und damit interdisziplinäre Therapiestrategien umso wichtiger. Die Erfahrungen des interdisziplinären Managements am Universitätsklinikum Regensburg im Zeitraum 1992 bis 10/2001 bei 145 HCC-Patienten werden dargestellt. 29 Patienten (20 %) wurden einer Resektion (22 Patienten) oder Lebertransplantation (7 Patienten) zugeführt. Kleinere, nicht resektable HCCs wurden mittels perkutaner Alkoholinjektion (PEI, n = 6) oder neuerdings Radiofrequenzthermoablation (RITA, n = 7) behandelt. Bei 48 Patienten mit fortgeschritteneren HCCs wurden 123 transarterielle Chemoembolisationen (TACE) alleine oder in Kombination mit PEI/RFA durchgeführt. 55 Patienten, die keiner operativen oder interventionellen Therapie zugeführt werden konnten, erhielten eine symptomatische Therapie. Anhand des Patientengutes der Universitätsklinik Regensburg sollen das interdisziplinäre Behandlungskonzept, seine Limitationen, Ergebnisse, Komplikationen und die Perspektiven dargestellt und diskutiert werden.
Abstract
Although HCC is one of the most frequent malignancies in the world, successful curative treatment is limited and interdisciplinary therapeutic approaches are required. Hereby, we present the experience of interdisciplinary management of HCC patients of the University Hospital of Regensburg from 1992 - 2001. In 145 patients HCC was diagnosed. 20 % of these patients were eligible for curative surgical treatment (resection: 22 patients, LTX 7: patients). Smaller, non-resectable HCCs were treated by percutaneous ethanol injection (PEI, n = 6) or more previously by radio frequency thermo ablation (RFA, n = 7). In 48 patients with advanced HCC, 123 TACE treatments were performed alone or in combination with PEI/RFA. Patients not eligible for these treatments received supportive care (55 patients, including non follow up-patients). The interdisciplinary therapeutic concept as well as the limitations, complications, outcome and potential coming improvements of these treatments are reported.
