Der „Superior Gluteal Artery Perforator Flap“ (S-GAP-Flap)

Zusammenfassung

Der obere Gluteallappen als freies Transplantat zur Brustrekonstruktion mit autologem Gewebe (S-GAP-Flap): Mit der Einführung der Perforanslappen ist zur Brustrekonstruktion mit autologem Gewebe die Glutealregion als Spenderareal neu untersucht worden. Mit dem S-GAP-Flap lässt sich eine Brust allein aus Fettgewebe und Haut der Gesäßregion rekonstruieren, ohne dass Anteile des M. glutaeus maximus zerstört werden müssen. Von 1997 bis 2002 wurden in der Klinik für Plastische und Handchirurgie des Behandlungszentrums Vogtareuth 52 S-GAP-Flaps zur Brustrekonstruktion mit einer Verlustrate von 7,7 % verwendet. Obwohl auch bei diesem Lappentransplantat eine deutliche Lernkurve mit initial höherer Verlustrate besteht, stellt es für uns heute eine zuverlässige Alternative zur Brustrekonstruktion ausschließlich mit autologem Gewebe dar, wenn das Unterbauchgewebe nicht zur Verfügung steht.

Abstract

Superior gluteal artery perforator flap for breast reconstruction with autologous tissue (S-GAP-flap): With the introduction of perforator flaps for breast reconstruction with autologous tissue the gluteal region became again investigated as a possible donor site. With the S-GAP-flap breast reconstruction from the gluteal region is possible utilising skin and fat without muscle sacrifice. 52 S-GAP-flaps for autologous breast reconstruction with a flap loss rate of 7.7 % were performed at the Department of Plastic and Handsurgery, Behandlungszentrum Vogtareuth, Germany from 1997 till 2002. Although there is an obvious learning curve also with this free tissue transfer, the S-GAP-flap is a reliable alternative in autologous breast reconstruction, when tissue from the lower abdomen is not available.
