Zusammenfassung
Die Klavikulafraktur ist eine der häufigsten knöchernen Verletzungen im Sport. Leistungs- und Profisportler riskieren erhebliche finanzielle Verluste, wenn sie in ihrer Disziplin bis zur Frakturheilung nicht antreten können. Diese Untersuchung stellt die Ergebnisse einer minimalinvasiven operativen Behandlung vor, die eine rasche Wiederaufnahme der sportlichen Aktivität erlaubt. In einem Zeitraum von 5 Jahren wurden 84 Klavikulafrakturen bei 80 Patienten mit einem elastischen Titannagel intramedullär stabilisiert. Unter den 80 Patienten befanden sich 12 Spitzensportler, 2 Frauen und 10 Männer im mittleren Alter von 24,8 ± 10 Jahren. Die Abduktion im Schultergelenk konnte von den Sportlern präoperativ im Mittel bis 36,3 ± 8° und postoperativ bis 154,2 ± 17° vorgeführt werden. Die subjektive Schmerzempfindung wurde von diesen Patienten präoperativ auf einer Visual-Analog-Skala (0 bis 100 Punkte) mit 71,7 ± 18 Punkten angegeben und lag damit signifikant höher als am 3. postoperativen Tag mit einem mittleren Punktwert von 19,2 ± 6. Nach der Operation nahmen die Sportler im Mittel nach 5,9 ± 1 Tage ihr Training wieder auf. An Wettkämpfen wurde im Mittel nach 16,8 ± 5 Tagen wieder teilgenommen. Alle Frakturen heilten. Der mittlere Wert des Constant-Score 12 Monate nach der Metall-entfernung betrug bei den Sportlern 98,3 ± 2 Punkten. Die elastisch stabile intramedulläre Osteosynthese der Klavikulafraktur bietet Sportlern die Chance einer raschen Wiederaufnahme von Training und Wettkampf und sollte den Betroffenen als Alternative zur konservativen Behandlung angeboten werden. Der sportliche und nicht zuletzt der finanzielle Schaden für den Betroffenen und dessen Sponsoren können hierdurch gering gehalten werden.
Abstract
In sportsmen, one of the most frequently observed injuries is a midclavicular fracture. As the absence from their discipline until the time of fracture healing comprises the risk of significant financial loss, the fracture of the clavicle is of prominent importance to top-performance and professional sportsmen. This study presents the results of a minimally invasive operative treatment of midclavicular fractures in patients working as top-performance or professional sportsmen. Within five years the intramedullary osteosynthesis was applied to 80 patients with 84 fractures of the clavicle. Out of these patients, 12 were top-performance or professional sportsmen. These 12 patients - 2 women and 10 men - were aged 24.8 ± 10 years. Preoperatively, the sportsmen presented an abduction of the shoulder joint averaging out at 36.3 ± 8°, postoperatively, averaging out at 154.2 ± 17°. On a visual-analog scale (0 to 100 points), the subjective pain, preoperatively, revealed 71.7 ± 18 points. This was found to be significantly higher than the subjective pain on the third day after surgery: 19.2 ± 6 points. After the operation the sportsmen resumed their training, on average, after 5.9 ± 1 days. Attendance at contests was, on average, continued after 16.8 ± 5 days. All of the sportsmen's fractures healed. Twelve months after implant removal, the sportsmen's average Constant-Score revealed 98.3 ± 2 points. The elastic stable intramedullary osteosynthesis of midclavicular fractures offers sportsmen the chance of quickly resuming training and competition. This treatment should be offered to the patients as an alternative to conservative treatment. Training stops and financial losses of the person concerned and of the sponsors can be minimised.
