Akute und chronisch entzündliche Neuropathien - Therapie[1]

Für das akute Guillain-Barré-Syndrom (GBS) ebenso wie die chronisch inflammatorische demyelinisierende Polyneuropathie (CIDP) wird pathogenetisch eine fehlgeleitete humorale und zelluläre Immunreaktion gegen im Einzelnen noch nicht identifizierte Epitope im peripheren Nerv, insbesondere den Nervenwurzeln und distaler Abschnitte, diskutiert. Demzufolge richtet sich die Behandlung in der akuten Phase des GBS und bei der oft schubförmigen Verschlechterung der CIDP gegen eine überschießende Entzündungsreaktion. Die Immuntherapie des GBS und der CIDP wurde in den vergangenen Jahren in einer Reihe von kontrollierten Studien evaluiert, die in dieser Übersicht zusammengefasst werden.
