Kutane Manifestationen des Lupus erythematodes

Zusammenfassung

Kutane Veränderungen treten bei bis zu 85 % aller Patienten mit Lupus erythematodes (LE) auf. Die Erhebung des Hautstatus sowie die Einordnung und Interpretation der Befunde stellt daher eine häufige Herausforderung für den behandelnden Arzt dar. Eine genaue Einteilung der Erkrankung ist eine wesentliche Voraussetzung für eine zielgerichtete Therapie, da verschiedene LE-Manifestationen mit unterschiedlichen Verläufen und Prognosen für die Patienten einhergehen und deshalb verschiedene, individuell angepasste, Therapiestrategien sinnvoll sind.

Es werden generell „spezifische” kutane Manifestationsformen, das heißt LE-Typen die für einen definierten Subtyp charakteristisch sind, von „unspezifischen” Formen unterschieden (Hierbei handelt es sich um Symptome die im Rahmen jeder LE-Manifestation auftreten können und somit nicht für einen speziellen Subtyp charakteristisch sind). Die spezifischen kutanen Läsionen werden üblicherweise in chronische (CCLE), subakute (SCLE) und akuet (ALE) unterteilt, wobei wiederum verschiedene Suptypen (z. B. chronisch diskoider LE, LE tumidus, LE profundus) unterschieden werden.

Wir stellen die Klassifikation dar und diskutieren die klinische Relevanz kutaner LE-Manifestationen vor dem Hintergrund der bekannten Literaturdaten sowie eigener Ergebnisse.
