Abstract
Aim
The Posterior Midsagittal Approach (PMA), originally used for the treatment of anorectal malformations, provides a wide exposure of the pelvic floor. Aim of this study is to describe and discuss the PMA in the treatment of intrapelvic malignant tumours which could not be radically excised by laparotomy alone.
Methods
Since 1997, four children were operated upon for pelvic Ewing's sarcoma, presacral sarcoma, intrapelvic neuroblastoma and prostatic rhabdomyosarcoma, respectively. The age of the patients ranged from two to eleven years. Two of them were males and two were females. In all patients, a laparotomy was performed, but inability to remove the tumour radically using this approach led to PMA. A midline incision extending between the tip of the coccyx and the posterior margin of the anus was made. The rectum was mobilised and retracted to the right side, using traction slings. Exposure of the lower part of the tumours was adequate, thus allowing them to be dissected in the midline anteriorly to the sacrum.
Results
In the patient with neuroblastoma diffuse bleeding was controlled by packing, which was removed on the 3rd postoperative day. Otherwise the postoperative course in all patients was free of complications. The patient with presacral sarcoma died one year after the operation from generalised disease. The remaining three patients are free of disease and continent two to six years postoperatively.
Conclusion
PMA is a safe procedure enabling radical excision of intrapelvic malignant tumours which are too high to be approached through the perineum and too low to be reached by laparotomy alone. The posterior midsagittal incision and lateral traction of the rectum permits easy access to the tumour and radical excision, without disturbance of the sphincteric function. Colostomy is not necessary.

Objectif
La voie postérieure sagittale moyenne (PSM), initiallement mise au point pour le traitement des malformations anorectales, permet une ouverture large du plancher pelvien. L'objectif de cette étude était de décrire la voie PSM et son application dans le traitement es tumeurs malignes intrapelviennes inextricables uniquement par laparotomie.
Méthodes
Depuis 1997 nous avons utilisé la voie PSM chez quatre enfants présentant respectivement un sarcome d'Ewing, un sarcome présacré, un neuroblastome intrapelvien et un rhabdomyosarcome prostatique. L'âge des enfants variait de deux à onze ans. La voie PSM était utilisée après échec de la résection radicale par laparotomie. L'incision médiane était tracée de l'extrémité du coccyx jusqu'à la marge postérieure de l'anus. Une mobilisation du rectum à droite a permis une exposition adéquate de la partie inférieure de la tumeur. La résection était réalisée par dissection médiane en avant du sacrum.
Résultats
Chez le patient avec un neuroblastome, une hémorragie diffuse était contrôlée par un pansement compressif retiré le troisième jour. Aucune complication postopérative n'était constatée. Le patient avec un sarcome présacré est décédé un an après l'intervention suite à une dissémination généralisée. Les trois autres patients sont sans récidive deux à six ans après l'intervention.
Conclusion
La sécurité de la voie PSM permet une résection radicale des tumeurs malignes intrapelviennes situés trop haut pour une résection transpérinéale et trop bas pour une résection uniquement par laparotomie. L'incision postérieure sagittale moyenne et la traction latérale du rectum facilitent l'accès au tumeur et une résection radicale, sans perturber la fonction sphinctérienne. Une colostomie n'est pas nécessaire.

Objetivo
El abordaje sagital posterior usado originalmente para el tratamiento de las malformaciones anorectales proporciona una exposición amplia del suelo pélvico. El objetivo de este estudio es describir y discutir el abordaje sagital posterior para el tratamiento de los tumores malignos de la pelvis que no pueden ser extirpados radicalmente solamente a través de una laparotomía.
Métodos
Desde 1997 hemos operado 4 niños por sarcoma de Ewing, sarcoma presacro, neuroblastoma intrapélvico y rabdomiosarcoma prostático respectivamente. La edad osciló entre los 2 y los 11 años. Dos de ellos eran niños y 4 niñas. En todos se realizó una laparotomía pero la imposibilidad de extirpar radicalmente el tumor a través de esta vía nos condujo al abordaje posterior. Se realizó una incisión media entre la punta del coxis y el margen posterior del ano. Se movilizó el recto y se retrajo hacia el lado derecho usando cintas de tracción. La exposición de la parte baja de los tumores fue adecuada permitiéndonos su disección en la línea media por delante del sacro.
Resultados
En el paciente con neuroblastoma hubo que controlar un sangrado difuso con packing que se retiró al tercer día. Por lo demás el curso postoperatorio en todos los pacientes no tuvo complicaciones. El enfermo con sarcoma presacro murió un año después de la operación de enfermedad generalizada. Los demás pacientes están libres de enfermedad y continentes 2 a 6 años después de la operación.
Conclusión
El abordaje sagital posterior es un procedimiento seguro que permite la extirpación radical de tumores malignos intrapélvicos demasiado altos para ser abordados por vía perineal y demasiado bajos para hacerlo a través de una laparotomía. La incisión sagital posterior y la tracción lateral del recto permiten un acceso fácil al tumor y una excisión radical sin alteración de la función esfinteriana. No hace falta colostomía.

Der posteriore midsagittale Zugang, der ursprünglich der Behandlung anorektaler Fehlbildungen vorbehalten war, daher einen weiten und ausgezeichneten Überblick über den Beckenboden erlaubte, wird in der vorliegenden Studie auf intrapelvine Malignome angewandt, die durch eine Laparotomie alleine nicht radikal entfernt werden konnten.
Methodik
Seit 1997 wurden 4 Kinder mit einem intrapelvinen Ewingsarkom, einem präsakralen Sarkom, einem intrapelvinen Neuroblastom und einem prostatischen Rhabdomyosarkom operiert. Das Alter der Patienten schwankte zwischen 2 und 11 Jahren. Zwei Patienten waren Knaben, 2 Mädchen. Bei allen Kindern wurde eine Laparotomie durchgeführt. Der Tumor konnte jedoch hierdurch nicht radikal entfernt werden, so dass zusätzlich eine posteriore sagittale Freilegung erfolgen musste. Der Zugang erfolgte in der Mittellinie von der Kreuzbeinspitze bis zum Oberrand des Anus. Das Rektum wurde mobilisiert und nach der Seite verlagert, nachdem es angeschlungen worden war. Auf diese Weise konnte der untere Anteil des Tumors ausgezeichnet dargestellt und reseziert werden.
Ergebnisse
Bei dem Patienten mit dem Neuroblastom kam es zu diffusen Blutungen, die jedoch durch Tamponade zum Stillstand gebracht werden konnten. Die Tamponade wurde am 3. postoperativen Tag entfernt. Bei allen anderen Kindern kam es zu keinen Komplikationen. Bedauerlicherweise starb das Kind mit dem präsakralen Sarkom ein Jahr nach der Operation aufgrund einer Generalisierung der Erkrankung. Die übrigen 3 Patienten sind 2 - 6 Jahre nach dem operativen Eingriff gesund und kontinent.
Schlussfolgerungen
Der posteriore, midsagittale Zugang ist eine sichere, ausgezeichnete Methode, um per Laparotomie nicht resezierbare, intrapelvine, maligne Tumoren zu entfernen. Entscheidend ist, das Rektum zur Seite zu halten, was einen ausreichenden Zugang zum Tumor gewährleistet und keinen Einfluss auf die Schließmuskelfunktion hat. Eine Kolostomie ist nicht notwendig.
