Zusammenfassung
Hintergrund: Erhöhte Temperaturen im Bereich von 40 - 44 °C bewirken eine Wirkungsverstärkung verschiedener zytostatischer Medikamente wie z. B. Stickstoff-Lost-Derivate, zytotoxischer Antibiotika und Platin-Analoga. Klinisch wird die Thermo-chemotherapie (TCH) mit dem Ziel eingesetzt, eine komplette Tumorresektion zu erleichtern und die lokale Tumorkontrolle zu verbessern. Patienten und Methoden: Zwischen 07/1993 und 12/2002 wurden 39 Patienten in eine Phase-II-Studie aufgenommen (weiblich = 24, männlich = 15, Alter 1 - 37,5 J., median 5,2). Darunter befanden sich 24 Patienten mit nicht-testikulären Keimzelltumoren und 15 Patienten mit Weichteilsarkomen oder Chondrosarkomen. Die Indikation für den Einsatz der TCH war: Loko-regionales Rezidiv (n = 29) oder ein nicht-resektabler Tumor nach neoadjuvanter Chemotherapie (n = 10). Insgesamt fand sich bei 10 Patienten ein unzureichendes Ansprechen oder eine Tumorprogression unter Primär- oder Rezidivchemotherapie. Bei 10 der Rezidiv-Patienten hat es sich um ein Mehrfach-Rezidiv gehandelt. Die Tumorlokalisation war: Becken (30), Abdomen (4), Kopf/Hals (2), Oberschenkel (2) und Lendenwirbelsäule (1). Thermochemotherapie (TCH): 1800 - 2000 mg Ifosfamid/m2 und 100 mg Etoposid/m2 an den Tagen 1 - 4 sowie 40 mg Cisplatin/m2 an den Tagen 1 + 4 in Kombination mit regionaler Tiefenhyperthermie (42 - 44 °C, 1 h) an den Tagen 1 + 4. Ergebnisse: Bei 39 Protokollpatienten sind insgesamt 166 TCH-Kurse mit 332 Hyperthermiebehandlungen zur Anwendung gekommen. Bei zwanzig Patienten hat die TCH zu einer kompletten Remission geführt, und bei neun Patienten ist eine partielle Remission erreicht worden. TCH ist bei 28/39 Patienten durch operative Maßnahmen und/oder Radiotherapie bei 13/39 Patienten ergänzt worden. Bei einer medianen Nachbeobachtungszeit von 27 Monaten liegt das ereignisfreie Überleben bei 0.39 ± 0.11 (20/39 Patienten) und das Gesamtüberleben bei 0.52 ± 0.11 (25/39 Patienten). Der aktuelle Status der Patienten dieser Hochrisikokohorte ist: 22 NED, 3 AWD, 12 DOD, 2 DOC. Schlussfolgerung: TCH stellt eine effektive Therapiemodalität bei nichtresektablen soliden Tumoren dar, die eine vollständige Tumorresektion bei 14 von 28 operierten Patienten ermöglicht hat. Bei einem Einsatz der TCH in einem multimodalen Therapiekonzept kann in Kombination mit operativen Maßnahmen und/oder Bestrahlung bei der Mehrzahl der Patienten mit loko-regionalen Rezidiven eine anhaltende Remission erreicht werden. Der therapeutische Effekt ist am größten, wenn die TCH im ersten Rezidiv eingesetzt wird. Aufgrund der klinischen und histopathologischen Heterogenität dieser seltenen Patienten ist eine validere Auswertung bezüglich des therapeutischen Stellenwerts der TCH nur im Rahmen einer Paar-Analyse in Zusammenarbeit mit den jeweiligen Studienzentralen der Therapieoptimierungsprotokolle möglich.
Abstract
Background: Elevated temperatures of 40 - 44 °C increase the actions of various anticancer drugs including N-lost derivatives, cytotoxic antibiotics and platinum analoga. In clinical usage thermochemotherapy (TCH) should facilitate surgical resection and ameliorate local tumor control. Patients and Methods: From 07/1993 to 12/2002 a total of 39 patients have been enrolled onto a phase-II study (female = 24, male = 15, age 1 - 37.5 years, median 5.2). Among these, 24 patients had extracranial non-testicular germ cell tumors and 15 patients soft tissue or chondrosarcomas. Indication: locoregional relapse (n = 29) or unresectable tumor after neoadjuvant chemotherapy (n = 10). Among these two groups, there were ten patients with poor response or progressive disease under primary or relapse chemotherapy. Ten out of the 29 relapse patients had more than one relapse. Tumor site: pelvis (30), abdomen (4), head and neck (2), proximal leg (2) and lumbar spine (1). Thermochemotherapy (TCH): 1800 - 2000 mg ifosfamide/m2 and 100 mg etoposide/m2 on days 1 - 4 and 40 mg cisplatin/m2 on days 1 + 4 combined with regional deep hyperthermia (42 - 44 °C, 1 h) on days 1 + 4. Results: In 39 protocol patients a total of 166 TCH courses (332 heat sessions) were applied. 20 patients achieved complete response, and 10 patients achieved partial response. TCH was followed by surgical tumor resection in 28/39 patients and/or radiotherapy in 13/39 patients. At a median follow-up of 27 months, outcome in this high-risk patient population was 22 NED, 3 AWD, 12 DOD, 2 DOC. Five year event free (EFS) and overall survival (OS) for the whole study cohort was 0.39 ± 0.11 (20/39 patients) and 0.52 ± 0.11 (25/39 patients), respectively. Conclusion: TCH shows substantial therapeutic efficacy and facilitates complete tumor resection in 14 out of 28 operated patients. Multimodal treatment including TCH, surgical resection and/or radiotherapy leads to sustained remission in the majority of patients with locoregional tumor recurrence. The therapeutic effect is most pronounced, if TCH is administered at first relapse. Due to the clinical and histologic heterogeneity the number of patients eligible for TCH is limited. Therefore, a more valid assessment of treatment efficacy can only be made by a matched-pair comparison in cooperation with the clinical registers.
