Komatöser Patient nach Fenstersturz

Zusammenfassung

Es wird über einen 30-jährigen Patienten berichtet, der bewusstlos unterhalb eines im 2. Stock geöffneten Fensters aufgefunden wird. Bei fehlenden Verletzungszeichen, jedoch sich auf Schmerzreize ungezielt versteifender Muskulatur - interpretiert als schweres geschlossenes Schädel-Hirn-Trauma - wird der Patient intubiert und beatmet. In der radiologischen Diagnostik zeigt sich jedoch keine intrakranielle oder schwere extrakranielle Verletzung. Zwei Stunden später erwacht der Patient ohne persistierendes neurologisches Defizit. Weitere Diagnostik enthüllt eine akute katatone Krise bei wahnhafter Schizophrenie als zugrunde liegende Krankheit. Diese Kasuistik unterstreicht die Notwendigkeit, nichttraumatische Ursachen als Ursachen für Bewusstlosigkeit zu berücksichtigen. Die Ausbildung von Notärzten für psychiatrische Notfälle wird diskutiert.

Abstract

A report is given on the case of an 30-year old patient found unconscious after defenestration out of 2^nd stair. Because of unsightet reaction and hardening of muscles on pain - interpreted as symptoms of severe closed head injury - intubation and controlled ventilation were performed in the field. Radiologic examination in the hospital, however, revealed no evidence of intracranial or severe extracranial injury. Two hours later the patient regained consciousness without any persistend neurologic deficit. Further diagnostic workup revealed catatonia based on schizophrenia as the underlying disease. The presented case underlines the necessity of considering non-traumatic causes of sudden loss of consciousness. Education and training of emergency physicians in psychiatric emergencys are discussed.
