Zusammenfassung
Die Manipulation ist ein adäquates Behandlungsverfahren bei akuten Blockierungen. Um lokalisierte oder generalisierte Knochenerkrankungen auszuschließen, sollte vor dem Einsatz von Manipulationen stets ein Röntgenbild des betroffenen Bereichs angefertigt werden.
In diesem Fallbeispiel wurde trotz der bekannten Bence-Jones-Proteinurie anscheinend kein Röntgenbild angefertigt, bei dem die deutliche Osteopenie des Achsenskeletts aufgefallen wäre. Die endgültige Klärung, ob es sich hierbei um eine pathologische oder traumatische BWK-7-Fraktur handelte, ließ sich nicht zweifelsfrei herbeiführen, da sich der Patient nicht einer transpedikulären Stanzbiopsie unterziehen wollte.
Abstract
Manipulation is an adequate treatment intervention for acute functional locking. In order to exclude local or generalised bone diseases, a X ray of the affected region should always be taken before the application of manipulation.
In this case report, although Bence Jones proteinuria was already known, X rays were apparently not taken. These would have revealed the distinct vertebral osteopenia. Definitive clarification of whether the fracture of the 7th thoracic vertebra was pathological or traumatic could not be verified beyond doubt as the patient decided against a transpedicular biopsy.
