Zusammenfassung
Fragestellung
Wir berichten über die diagnostischen und therapeutischen Methoden bei 52 Schwangeren mit symptomatischer Harnstauungsniere und diskutieren den Einsatz der Kernspintomographie (NMR) bei der Diagnostik der Urolithiasis sowie die unterschiedlichen Therapieoptionen.
Methodik
Zwischen Januar und Dezember 2000 behandelten wir in unserer Abteilung 52 Schwangere mit symptomatischer Harnstauungsniere. Der Krankheitsverlauf wurde anhand der Akten retrospektiv analysiert.
Ergebnisse
34 der 52 Patientinnen erhielten zur Entlastung der gestauten Niere eine DJ-Harnleiterschiene in Lokalanästhesie unter sonographischer Kontrolle. Alle Schwangeren waren innerhalb einiger Tage beschwerdefrei. Lediglich einmal musste bei DJ-Unverträglichkeit ein perkutanes Nephrostoma eingelegt werden. Bei zwei Schwangeren mit Steinanamnese und typischer Klinik führten wir auf Wunsch der Patientinnen zunächst eine NMR-Untersuchung durch. Bei Nachweis jeweils eines distalen Harnleitersteines erfolgte in Kurznarkose eine primäre Ureteroskopie mit Steinextraktion. Da diese Behandlungen sowie die weitere Schwangerschaft ohne Komplikationen verliefen und ein postpartal durchgeführtes i.v. Urogramm in beiden Fällen Steinfreiheit zeigte, befürworten wir bei distalen Harnleitersteinen die primäre Ureteroskopie mit Steinextraktion durch einen erfahrenen Operateur. Sie bleibt allerdings wegen der zeit- und kostenintensiven NMR-Untersuchung auf ausgewählte Einzelfälle beschränkt.
Abstract
Purpose
We present here the diagnostics, therapy and course of 52 patients with symptomatic hydronephrosis during pregnancy and discuss the use of nuclear magnetic resonance tomography in the diagnosis of urolithiasis.
Methods
Between January and December 2000 we treated 52 pregnant women with symptomatic urolithiasis. Diagnostics, therapy and further course were analyzed retrospectively.
Results
34 out of the 52 patients were given a ureteral DJ stent under local anesthesia for renal drainage; placement of the stent was carried out under sonographic monitoring. All patients were free of complaints within a few days. Only in one case did it prove necessary to implant a percutaneous nephrostoma because of DJ stent intolerance. In two women with diagnosis of ureteral stone and typical clinical symptoms we performed a NMR tomography at the request of the patients. After detection of a distal ureteral stone in both of the patients, we carried out a primary ureteroscopy under short anesthesia with subsequent removal of the ureterolith. As both treatments were performed without any complications occurring and an i.v. urogram after termination of the pregnancy showed no indications of any further stones in either of the patients, we recommend primary ureteroscopy with removal of the ureterolith after careful prior investigation. Nevertheless, as the procedure is both time-consuming and cost-intensive, NMR tomography should be limited to selected individual cases.
