Abstract
Purpose: A wide variety of sclerosing agents have been used in the treatment of rectal prolapse (RP) in children. We have used 15 % saline solution for the first time in the treatment. The aim of this study is to review the results of a 15 % saline solution and other sclerosing agents.
Patients and Methods: A total of 16 children with RP were treated by injection of 15 % saline solution. Under general anesthesia, the patient was placed in the lithotomy position. The left index finger was inserted into the rectum to control the position of the needle, a 20-gauge spinal needle was introduced through the perianal skin and was advanced. The saline was slowly injected, the needle was then withdrawn slightly, and the injection was continued until 2 - 3 ml of 15 % saline were injected. The injection was made into the submucosal tissue, the right perirectal area, the left perirectal area, and posterior to the rectum at 5 points.
Results: Conservative treatment had previously failed in all patients. Prolapse ceased in 15 (93.7 %) of the 16 children after the first injection. Only one patient required a second injection. There were no complications.
Conclusions: The success rates and complications of the treatment reported in the literature differ for each sclerosing agent. 15 % saline is preferable because of the high cure rate, the safety of the procedure, the easy injection, and the lack of complications.

Objectif: De nombreux agents sont utilisés pour la sclérothérapie du prolapsus rectal chez l'enfant. Nous avons utilisé pour la première fois une solution saline de 15 p.100. L'objectif de cette étude est de faire le point sur les résultats obtenus avec des différents agents sclérosants dont une solution saline de 15 p.100.
Patients et Méthodes: Seize enfants présentant un prolapsus rectal étaient traités par injection d'une solution saline de 15 p.100 sous anesthésie générale en position de lithotomie. La position de l'aiguille était contrôlée au doigt (index gauche) inséré dans le rectum avant l'introduction transcutanée périanale d'une aiguille spinale n° 20. La solution était injectée progressivement par cinq points dans la sous-muqueuse au niveau périrectal droit, gauche et postérieur retirant légèrement l'aiguille pour poursuivre l'injection d'un total de 2 - 3 ml de solution saline.
Résultats: Un traitement conservateur préalable était un échec chez tous les patients. La résolution du prolapsus était obtenue chez 15 (93,7 p.100) des 16 patients après la première injection. Une deuxième injection était nécessaire pour un patient. Nous n'avons pas noté de complication.
Conclusion: Les taux de succès et de complication rapportés dans la littérature sont très variables en fonction de l'agent sclérosant utilisé. Nous préférons la solution saline de 15 p.100 en raison du taux du succès élevé, de l'innocuité de l'intervention, de la facilité de l'injection et de l'absence de complication.

Objetivo: Se han usado varios agentes esclerosantes para el tratamiento del prolapso rectal (RP) en niños. Nosotros hemos usado por primera vez solución salina al 15 %. El objetivo de este estudio es revisar los resultados de este tratamiento y compararlos con los de otros agentes esclerosantes.
Pacientes y Métodos: Hemos tratado 16 niños con RP mediante inyección de solución salina al 15 %. Bajo anestesia general con el paciente en posición de litotomía se introdujo el índice izquierdo en el recto para controlar la posición de la aguja del 20 introducida a través de la piel perianal. Se inyectó lentamente el suero salino retirando progresivamente la aguja mientras se continuaba la inyección hasta un total de 2 a 3 ml. La inyección se llevó a cabo en los tejidos submucosos, en la región perirrectal derecha, en la región perirrectal izquierda y posteriormente al recto en los 5 puntos.
Resultados: El tratamiento conservador había fallado en todos los pacientes. Desapareció el prolapso en 15 de los 16 niños (93,7 %) tras la primera inyección. Solamente uno requirió una segunda inyección. No hubo complicaciones.
Las tasas de éxito y las complicaciones del tratamiento publicadas en la literatura fueron diferentes para otros agentes esclerosantes. Debe preferirse el salino al 15 % a causa de la alta tasa de curación, la facilidad del tratamiento y la ausencia de complicaciones.
Zusammenfassung
Zielsetzung: In der Behandlung des Rektumprolapses im Kindesalter wurde eine Vielzahl sklerosierender Agenzien eingesetzt. Wir haben 15 %ige Kochsalzlösung als Primärtherapie verwandt und stellen unsere Ergebnisse in der vorliegenden Arbeit vor.
Material und Methodik: Insgesamt 16 Patienten mit Rektumprolaps wurden mit 15 %iger Kochsalzlösung behandelt. In Allgemeinanästhesie und Steinschnittlage unter Führung des linken Zeigefingers wurde die Nadel in den Sphinkter eingebracht und mittels einer Spinalnadel durch die perianale Haut die Kochsalzlösung unter leichtem Zurückziehen der Nadel kontinuierlich (2 - 3 ml) injiziert. Die Injektionsorte waren das submuköse Gewebe der rechten und linken Perirektalgegend sowie im Bereich der Rektumhinterwand an 5 Stellen.
Ergebnisse: Bei allen Patienten hatte die konservative Therapie versagt. Nach der ersten Injektion verschwand der Prolaps bei 93,7 % der Kinder (15 von 16). Nur ein Patient benötigte eine zweite Injektion. Komplikationen wurden nicht beobachtet.
Schlussfolgerungen: Die Erfolgsrate nach 15 %iger NaCL-Injektion ist außerordentlich hoch. Die Sklerosierungstherapie mit 15 %iger NaCL-Lösung kann daher als sehr erfolgreich, sicher und einfach empfohlen werden.
