Das ausgedehnte spinale epidurale Hämatom - Eine seltene Komplikation nach chirotherapeutischer Manipulationsbehandlung der Halswirbelsäule

Zusammenfassung

Einleitung: Das spinale epidurale Hämatom ist eine sehr seltene aber schwerwiegende Komplikation nach chirotherapeutischer Manipulation der Wirbelsäule. Bisher wurden in der Literatur lediglich drei Einzelfälle beschrieben, die immer eine chirurgische Intervention erforderten. Kasuistik: Bei einer 27-jährigen Patientin wurden eine chirotherapeutische Maßnahme der Halswirbelsäule (C5/6) und anschließend eine Facettengelenkinfiltration C5/C6 durchgeführt. Nach 10 Minuten kam es zu Hirndruckzeichen mit Übelkeit, Schwindel, Kopfschmerzen und Erbrechen. Das MRT zeigte eine epidurale Blutung, die sich von HWK1 bis zum Os sacrum ausdehnte. Auf eine operative Intervention konnte verzichtet werden, da motorische und sensible Defizite fehlten. Die Patientin konnte nach wenigen Tagen symptomfrei aus der stationären Behandlung entlassen werden. Schlussfolgerung: Nach chirotherapeutischer Manipulation und Entstehung neurologischer Symptome sollte ein spinales epidurales Hämatom in Betracht gezogen werden, um eine rasche und adäquate Therapie einleiten zu können. In unserem Fall kam es zu einer außergewöhnlich langstreckigen bisher noch nicht beschriebenen Hämatomausdehnung nach chirotherapeutischer Therapie. Trotzdem konnte aufgrund des vergleichsweise harmlosen Verlaufes im Gegensatz zu den bisher beschriebenen Fällen auf eine chirurgische Intervention verzichtet werden.

Abstract

Introduction: Spinal epidural hematoma is a rare complication after chiropractical manipulation. In the literature, only three cases have been reported, which all necessitated surgical treatment. Case: A 27-year-old woman was treated with cervical chiropractical manipulation (C5/6) and facet joint infiltration. 10 minutes later the patient presented signs of intracranial pressure with nausea, vertigo, headache and vomiting. The magnetic resonance imaging of the spine demonstrated an epidural hematoma extending from the cervical to the sacral spine. As the patient had no sensible or motor deficits and recovered quickly, surgical treatment was not necessary . A few days later the patient had a complete persisting remission of symptoms. Conclusion: If neurological deficits occur after chiropractical manipulation, a spinal epidural hematoma should be considered to provide adequate therapy without delay. The current case report shows an unusual expansion of the hematoma which has not described so far after chiropractical manipulation. But, in contrast to the three cases reported before, a surgical intervention was not necessary.
