Zusammenfassung
Für den klinischen Praktiker ist es wichtig, in Grundzügen Vorstellungen über die neurochemischen Mechanismen bei psychischen Störungen und ihrer Therapie zu haben. Aktuelle Erklärungen für psychische Störungen aus dem Bereich der biologischen Psychiatrie zeigen mehrere Probleme: 1. Grundlegend ist die Gleichsetzung von Gehirnfunktionen mit psychischen Funktionen nicht unproblematisch. 2. Die Fixierung auf Gehirnorte der Störungen übersieht den zirkulären Charakter der neuronalen Strukturen des Gehirns. 3. Eine Fokussierung auf neurochemische Prozesse bei Vernachlässigung elektrophysiologischer Prozesse findet statt. 4. Psychische Krankheiten und ihre Therapie werden auf molekulare synaptische Mechanismen zurückgeführt. Auf diese Weise wird ein restringiertes und reduktionistisches Bild von psychischen Störungen gezeichnet. Es wird übersehen, dass auch in der Biologie eine immanente Notwendigkeit besteht, dynamische Systemmodelle zu bilden: Der Umstand der hohen Vernetzung der neurochemischen Systeme und die Befundlage, dass mehrere neurochemische Systeme bei einem einzigen psychiatrischen Syndrom verändert sind (gestörtes „Säftegleichgewicht”), verweist darauf, dass Techniken der Systemtheorie für ein adäquates theoretisches Verständnis nützlich sein können.
Es wird daher ein Weg gezeichnet, wie in Zukunft eine methodologisch fundierte Theorie- und Modellbildung in der (biologischen) Psychiatrie aussehen kann. Dazu wird die Technik des systemischen Modellierens in Grundzügen dargestellt und ein einfaches Modell des neurobiologischen Basiskonzepts zur Schizophrenie von Carlsson anhand einer einfachen Computersimulation diskutiert. Auf diese Weise kann die Aufgabe einer zukünftigen theorieorientierten empirischen Forschung erläutert werden. Es wird schließlich darauf hingewiesen, dass die Bildung „transdisziplinärer” Arbeitsgruppen die Theoriebildung der Psychiatrie verbessern könnte, indem sich an der Modellierung neben den empirischen Forschern auch Mathematiker (bzw. Systemtheoretiker) und Kliniker beteiligen.
Abstract
For the clinical practitioner it is important, to have some fundamental ideas about the neurochemical mechanisms in mental disorders and psychiatric therapy. Current explanations for mental disorders coming from the field of biological psychiatry show some problems: 1. The identification of mental states with brain functions is not unproblematic. 2. Focussing onto brain locations of the disorders neglects the circular character of the neural structures of the brain. 3. Focussing on neurochemical processes disregard of electrophysiological processes occurs. 4. Mental disorders and their therapy are ascribed to molecular synaptic mechanisms. In this way a restricted and reductionistic picture of mental disorders arises. It is overlooked that in biology an immanent necessity exists to build up dynamic system models: The feature of the brain to consist of highly interconnected neurochemical systems and the finding that several neurochemical systems are altered in one single psychiatric syndrome (disturbed “balance of juice”), indicates that techniques of system theory can be useful for an appropriate theoretical understanding.
Therefore a way is shown, how a methodologically grounded theory and model formation in (biological) psychiatry can be followed in future. For this purpose, the technique of systemic modelling is represented in basics and a simple model of the neurobiological basic concept for schizophrenia by Carlsson is discussed by means of a computer simulation. In this way the prospective task of a theory-oriented empirical investigation can be illustrated. It is finally pointed out that the formation of “transdisciplinary” working groups would be able to improve theory formation in psychiatry, when in addition to the empirical researchers also mathematicians (and/or systems theoreticians) and clinicians are involved in modelling.
