Abstract
This article describes how narratives have always played their part in medicine. Narratives are a thing apart from case reports. They are found in the realm of subjective experiences and stories about the illness of the patients themselves. The goal of the narrative approach is the physician's aid in sorting and evaluating the patient's experiences.
This demands sensitivity on the part of the physician, as he can decide when and where to intervene, while empathically listening to the patient's story, in order to ”repair” these stories. Since the physician himself may well have his own stories which might mesh with the patient's narrative, it is necessary to observe the impact of the patient's story on the therapist. To demonstrate this point we present an interactive case report in which the therapist is reminded of own, long forgotten experiences while listening to the patient's narrative.
Zusammenfassung
Dieser Artikel beschreibt wie Narrative (Geschichten) immer eine Rolle in der Medizin gespielt haben. Narrative befinden sich außerhalb der formellen medizinischen Fallvorstellung, und wir finden sie im Reiche der subjektiven Erzählungen der Patienten über ihre Krankheiten. Die narrative Annäherung enthält die Hilfe des Klinikers beim Ordnen der Krankheitserfahrungen des Patientens und dem Finden von ihrem Sinn.
Es ist eine Empfindlichkeit gefordert, weil beim empathischen Zuhören der Geschichten von Patienten der Arzt beschließen kann wann und wie er reagieren und eingreifen will, um die Geschichten „zu reparieren“.
Die Wirkung von den Geschichten von den Patienten auf den Arzt wird betrachtet, weil der Arzt/die Ärtztin seine/ihre eigene Geschichten haben können die oft mit der von den Patienten verflechtet werden. Zur Demonstration von diesem Punkt bringen wir eine interaktive Fall-Schilderung, in welcher die Geschichte des Patienten, beim Arzt Erinnerungen aus seiner eigenen Gechichte hervorgerufen haben, die er eine lange Zeit vergessen hatte.
