Indikationen und Behandlungsergebnisse der V.A.C.®-Therapie in der Gefäßchirurgie - State of the art in der Behandlung chronischer Wunden?

Zusammenfassung

Die Behandlung chronischer Wunden verursacht in Deutschland jährlich Kosten in Höhe von schätzungsweise 5 Mrd. €. Nahezu 10 % der Bevölkerung sind im Laufe ihres Lebens durch chronische Wunden betroffen, die hieraus resultierende Letalität beträgt 2,5 % [15] [25] [34]. Häufig findet sich der Gefäßchirurg in seiner täglichen Praxis mit therapieresistenten Wunden als Folge der diabetischen Gangrän, des neuropathisch infizierten Fußes, der chronisch venösen Insuffizienz oder der fortgeschrittenen peripheren arteriellen Verschlusskrankheit konfrontiert.
Die Vacuum assisted closure (V.A.C.®)-Therapie stellt ein modernes Verfahren zur Behandlung chronischer Wunden dar, welches in der Gefäßchirurgie in Deutschland zunehmend Verbreitung findet. Auch in manchem zunächst aussichtslos erscheinendem Fall kann hiermit die Majoramputation verhindert und die Mehrzahl chronischer therapieresistenter Wunden zur Abheilung gebracht werden. Aufgrund erster Erfahrungen mit der V.A.C.®- Therapie bei der Behandlung infizierter Gefäßprothesen im Stadium II und III nach Szilágyi bietet auch hier die V.A.C.®-Therapie bei richtiger Indikationsstellung eine erfolgsversprechende Option, um nicht nur die betroffene Extremität und das Leben des Patienten, sondern auch den infizierten Graft in situ zu erhalten. Ein „successful outcome” wurde durchschnittlich nach einer mittleren Behandlungsdauer von 21,3 Tagen in 73,8 % der Fälle erzielt.

Abstract

Almost 10 % of the population are concerned in the course of her life by chronic wounds, mortality resulting from this amount to 2.5 % [15] [25] [34]. The Vacuum assisted closure (V.A.C.®) therapy represents a modern procedure for the treatment of chronic wounds. Also in some first appearing offering no prospects cases hereby the Majoramputation can be prevented and the majority of chronic therapy-resistant wounds can be healed. Due to first experiences with the V.A.C.® therapy with the treatment of infected vascular grafts in the stage II and III after Szilágyi it also offers a success - promising option to concerne the extremity and the life of the patient and to receive but also the infected graft in situ.
