Welche Abortprophylaxe ist bei einem Antiphospholipid-Syndrom zu wählen?

Frage: Bei einer jetzt 30-jährigen Patientin mit systemischem Lupus erythematodes (SLE) in Remission war es mit 20 Jahren zu einem Abort in der 12. Schwangerschaftswoche (SSW) und mit 24 Jahren trotz der Gabe von low dose Acetylsalicylsäure (ASS) während der gesamten Schwangerschaft zu einer Totgeburt in der 36.SSW gekommen. Bei der Patientin waren sowohl das Lupusantikoagulans als auch hochtitrige Antikardiolipin-Antikörper vom IgG-Subtyp nachweisbar. Die Patientin hatte zum Vorstellungstermin keinerlei medikamentöse Therapie. Jetzt bestand erneut dringender Kinderwunsch. Welche Therapie ist zur Vermeidung geburtshilflicher Komplikationen zu wählen?
