Dünndarmvolvulus: eine seltene Komplikation bei der Koloskopievorbereitung

Zusammenfassung

Anamnese und klinischer Befund: Ein 69 Jahre alter Patient wurde 16 Stunden nach einer problemlos durchgeführten Koloskopie mit zunehmenden abdominellen Schmerzen, die schon während der Laxantiengabe vor Koloskopie aufgetreten waren, stationär aufgenommen.

Untersuchungen: Die abdominelle Untersuchung, Laborchemie und abdomensonographischen Befunde zeigten das Bild einer beginnenden Peritonitis.

Therapie und Verlauf: Intraoperativ fand sich ein Dünndarmvolvulus um einen Narbenstrang einer jahrelang zurückliegenden Appendektomie. Es erfolgte eine Dünndarmteilresektion mit End-zu-End-Anastomose, der postoperative Verlauf war unauffällig.

Folgerung: Bei schon während der Vorbereitung einer Koloskopie auftretenden Beschwerden sollte auch an seltene Krankheitsbilder wie einen Dünndarmvolvulus gedacht werden.

Summary

History and clinical findings: A 69-year-old patient was hospitalized 16 h after investigation of the colon (screening colonoscopy), suffering from abdominal pain, which had started during the laxative period the day before colonoscopy.

Investigations: Clinical and technical investigations showed signs of a peritonitis.

Treatment and course: By laparotomy a volvulus of the small intestine was found, caused by adhesions after appendectomy years before. A resection of a part of the small intestine got necessary, there were no problems during the postoperative course.

Conclusions: If pain appears during the laxative period before colonoscopy, rare complications like volvulus of the small intestine should be considered.
