Zusammenfassung
Ziele des Projektverbundes „Früherkennung und Frühintervention” des Kompetenznetzes Schizophrenie sind die Entwicklung eines evaluierten Instrumentes zur Abschätzung des individuellen Psychoserisikos, der Nachweis von potenziellen Vulnerabilitätsindikatoren und funktionellen Hirnabweichungen, die den Beginn einer schizophrenen Erkrankung anzeigen könnten, sowie die Entwicklung von präventiven Frühinterventionen bei Personen mit einem erhöhten Psychoserisiko. Erste Analysen zeigen, dass ein differentieller Interventionsansatz, bestehend aus kognitiver Verhaltenstherapie im psychosefernen Prodrom und stützender Intervention kombiniert mit atypischem Antispychotikum im psychosenahen Prodrom, viel versprechend ist. Obwohl die Datenlage noch nicht abschließend zu beurteilen ist, stimmen die Zwischenauswertungen und die vorliegenden Ergebnisse internationaler Studien optimistisch für die Entwicklung einer indizierten Prävention bei schizophrenen Störungen.
Summary
The aims of the early detection and intervention programme of the German Research Network on Schizophrenia are to develop an evaluated assessment instrument for the detection of persons at risk, to explore biological indicators, which could predict the development of schizophrenia and to develop treatment strategies for persons at risk of psychosis. First preliminary results of the differential intervention approach to the initial prodromal state - a comprehensive cognitive-behavioural therapy for persons in the early initial prodromal state and clinical management and atypical neuroleptics combined in persons in the late initial prodromal state - are promising. However, although we have to await the final results of the ongoing trials, the preliminary data and findings from international studies are suggesting that indicated prevention may become a new perspective in the treatment of schizophrenia.
