Einzelzahnimplantate - die optimale Therapie für fehlende seitliche Schneidezähne?

Zusammenfassung

Im Anschluss an das im American Journal of Orthodontics and Dentofacial Orthopedics (AJODO) veröffentlichte Editorial Treatment of missing lateral incisors [1] hält eine Diskussion an, die durch eine Erwiderung von Drs. Wilson und Ding [2] erst richtig angefacht wurde. Beide sehen osseointegrierte Implantate als optimales Behandlungsmittel für den Ersatz fehlender seitlicher Schneidezähne an. Unseres Erachtens werden die Langzeitergebnisse von implantatgestützten Kronen als zu optimistisch angesehen und keineswegs durch die einschlägige wissenschaftliche Literatur belegt. Obwohl sie Schwierigkeiten und Probleme mit 3-gliedrigen Brücken, festsitzendem Zahnersatz und Lückenschluss erwähnen, fehlen die Nachuntersuchungen von Einzelzahnimplantaten. Die Behauptung, dass ein orthodontischer Lückenschluss den Zahn und das Periodontium exponiert und zu einer lebenslangen Kontrolle und Pflege zwingt, trifft wohl eher auf den implantatgestützten Zahnersatz zu, wie die folgende Diskussion belegen soll. Der Ersatz fehlender seitlicher Schneidezähne im Oberkiefer durch osseointegrierte Implantate birgt viele potenzielle Probleme, so dass auch aufgrund der vorliegenden Langzeituntersuchungen zunehmend mehr Vorsicht geboten ist.
