Can Eyelashes Migrate?

Zusammenfassung

Hintergrund: Intraokulare Wimpern nach Trauma oder Operation können eine Endophthalmitis auslösen. Bei einem jungen Patienten ohne Traumaanamnese mit akuter intraokularer Entzündung kann die Diagnose leicht verpasst werden. Anamnese und Befund: Ein junger Bauer entwickelte eine akute Skleritis mit umschriebenem progredienten Netzhaut-, Aderhaut- und Glaskörperinfiltrat. Anamnese und Befunde ergaben keinen Hinweis auf eine Augenverletzung oder eine Systemerkrankung. Therapie und Verlauf: Im kultivierten Glaskörpermaterial wuchsen Staphylokokken. Im Rahmen der Glaskörperchirurgie wurde eine in der Netzhaut steckende Wimper identifiziert und entfernt. Nach intravitrealer antibiotischer Therapie verschwand die Entzündung und der Visus normalisierte sich. Schlussfolgerung: Bei einer akuten intraokularen Entzündung sollte ein intraokularer Fremdkörper in die Differenzialdiagnose mit eingeschlossen werden, auch wenn keine Hinweise auf ein Trauma vorliegen.

Abstract

Background: Intraocular cilia after penetrating injuries or surgery causing acute or subclinical inflammation are a well-known problem. In a healthy young patient with acute intraocular inflammation but no history of trauma the diagnosis may be missed initially. History and signs: A young farmer presented with severe eye pain, scleritis and a circumscribed chorioretinal and vitreous infiltrate. There was no history or evidence of eye trauma or systemic disease. Therapy and outcome: Staphylococci were identified from vitreous material. During vitreous surgery an intraretinal cilium was found and removed. After intravitreal antibiotic treatment, the vision completely recovered. Conclusion: Acute intraocular inflammation should alert the ophthalmologist to consider an intraocular foreign body as a possible cause even if there is no history of trauma.
