Zusammenfassung und Ausblick
Die chirurgische Stimmrehabilitation mit tracheo-ösophagealen Stimmfisteln und einer alloplastischen Prothese ist gegenwärtig in Deutschland Standard für laryngektomierte Patienten. Mit der zunehmenden Verbreitung dieser Rehabilitationsform ergeben sich nicht nur für die Kollegen in der Klinik, sondern auch für die Kollegen im niedergelassenen Sektor neue Aufgaben. Die heimatortnahe Versorgung dieser Patienten muss unser aller Ziel sein. Mit zunehmender Anwendung von Stimmprothesen stellen sich dem Anwender wenige, gut behandelbare Probleme, für die er ein Therapiekonzept bereithalten sollte. Die chirurgisch-plastische Stimmrehabilitation ist unter anderem wegen ihres hohen operativen Aufwandes nur für Einzelfälle indiziert. Wenn eine chirurgische Stimmrehabilitation nicht möglich ist oder gelingt, sind die Ösophagusersatzstimme und die elektronischen Sprechhilfen sinnvolle Alternativen. In der Perspektive sind Verbesserungen in der Haltbarkeitsdauer der Stimmprothesen, den Methoden des Stimmprothesenwechsels und dem fingerfreien Tracheostomaverschluss beim Sprechvorgang zu erwarten. Das große Ziel des künstlichen Kehlkopfes mit Rückverlegung des Tracheostomas sind Ziele in der weiteren Zukunft.
