Das Stimmdiagnostik-Protokoll der European Laryngological Society (ELS) - erste Erfahrungen im Rahmen einer Multizenterstudie

Zusammenfassung

Einleitung: Zur standardisierten Diagnostik von Stimmstörungen wurde von der ELS ein multidimensionales Protokoll vorgeschlagen. Um die Sinnhaftigkeit und Relevanz dieses Protokolls in der klinischen Routine zu evaluieren, wurden die Erfahrungen, die damit in 6 universitären europäischen Stimmzentren gewonnen wurden, retrospektiv ausgewertet.

Material und Methodik: Das ELS-Protokoll umfasst 5 Dimensionen: perzeptive Stimmklangbeurteilung, Videolaryngostroboskopie, aerodynamische Messungen, akustische Messungen und die subjektive Selbstevaluation der Stimmqualität bzw. der kommunikativen Beeinträchtigung durch den Patienten. Die Ergebnisse von 94 Patienten mit benignen Stimmerkrankungen wurden im Rahmen der Multizenterstudie retrospektiv ausgewertet.

Ergebnisse: Es konnte gezeigt werden, dass das vorgeschlagene multidimensionale ELS-Protokoll für alle „normalen” Stimmstörungen valide Ergebnisse von klinischer Relevanz zu liefern im Stande ist. Das Protokoll ist in der klinischen Routine anwendbar und praktikabel. Extreme stimmliche Abweichungen wie bei spasmodischer Dysphonie, Aphonie oder nicht laryngealen Ersatzstimmbildungen konnten nicht in allen Dimensionen reproduzierbar erfasst werden. Die 5 Dimensionen sind nicht redundant, und sie reflektieren die sehr unterschiedlichen Ätiologien der Stimmstörungen, Arten und Verlauf der Behandlung sowie auch geschlechtsspezifische Unterschiede.

Abstract

Introduction: A multidimensional protocol has been established by the ELS in order to reach better agreement and standardisation for functional assessment of pathologic voices. In order to evaluate the validity, practicability and applicability of this protocol the experiences of 6 european voice centres have been analysed in a retrospective study.

Material and Methods: The ELS protocol comprises 5 dimensions: perceptual voice evaluation, videostroboscopy, acoustics, aerodynamics and subjective rating by the patient. Results obtained in 94 patients with benign voice disorders were evaluated retrospectively in a multicenter study.

Results: According to our results, the validity, practicability and applicability of the ELS protocol was largely satisfactory. This was true for all ”common” voice disorders, but not for extreme voice alterations (e. g. spasmodic dysphonia, aphonia, substitution voices). The 5 dimension proofed to be not redundant and were able to selectively differentiate pre- post changes among various etiologies of voice disorders, various types of treatment and genders.
