Eine Alternative zur chirurgischen Intervention - Endoskopische Mukosaresektion früher gastrointestinaler Tumoren

Zusammenfassung

Die endoskopische Mukosaresektion (EMR) erlaubt die komplette Abtragung und damit eine Heilung früher Neoplasien des oberen und unteren Gastrointestinaltrakts. Voraussetzung dafür ist die exakte und rechtzeitige Diagnose der frühen gastrointestinalen Neoplasie, was eine Expertise sowohl des Endoskopikers als auch des Pathologen erfordert. Die endoskopische Mukosaresektion wird in Abhängigkeit von der Ausdehnung der Neoplasie, der Lokalisation im Gastrointestinaltrakt und der Erfahrung des endoskopierenden Untersuchers in verschiedenen technischen Variationen ausgeführt. In den meisten Studien erreicht die Rate der kompletten Abtragung etwa 80-95 %. Prinzipiell ist die endoskopische Mukosaresektion in der Hand des erfahrenen Untersuchers ein sicheres Verfahren mit einer niedrigen Komplikationsrate. Als typische Komplikationen finden sich Blutung, Perforation und seltener Strikturen. Mittlerweile ist das Verfahren eine etablierte Alternative zur chirurgischen Resektion. Verbesserungen der Technik werden in Zukunft die Ausdehnung der Indikationen erlauben.

Summary

Endoscopic mucosal resection (EMR) enables the complete and therefore curative resection of early neoplasias of the gastrointestinal tract. Special expertise of the endoscopist and pathologist are necessary to diagnose the early neoplasias of the GI-tract exactly. Dependent from the size of the early neoplasias and their localization in the gastrointestinal tract different techniques are used for endoscopic mucosal resection. Complete resection of early neoplasias is enabled in 80-95 % of cases. If it is performed by experienced and well-trained endoscopists EMR is generally safe and has a low complication rate. Typical complications are bleeding, perforation and stricture formation, occasionally. By now, endoscopic mucosal resection is a well-established alternative to surgical resection. Improvements of the EMR-technique will allow broader use of EMR in the future.
