In einem 8-Jahres-Zeitraum wurden 15 posttraumatische Pseudarthrosen des proximalen Humerus operativ mit einer Winkelplatte („95°-Kondylenplatte für kleinwüchsige Erwachsene und Jugendliche“, „durchbohrte Winkelplatte 90°“, „Adoleszentenplatte“) versorgt. 12 Patienten konnten durchschnittlich 23 Monate nach operativer Versorgung klinisch und radiologisch nachuntersucht werden. Das Durchschnittsalter der nachuntersuchten Patienten betrug 58 Jahre. In allen Fällen lag eine Pseudarthrose im Collum chirurgicum vor. Zum Zeitpunkt der Nachuntersuchung waren ⅔ der Patienten subjektiv mit dem Ergebnis der Operation zufrieden. In 11 Fällen kam es zu einer knöchernen Konsolidierung der Pseudarthrose. Gemäß dem UCLA- und dem Constant-Score erreichte die Hälfte der Patienten ein sehr gutes oder gutes Ergebnis. Insgesamt stellt die Winkelplattenosteosynthese ein wertvolles Verfahren zur operativen Behandlung in den schwierigen Problemfällen der Pseudarthrose des Collum chirurgicum am proximalen Humerus dar.
Abstract
We reviewed the medical records, operative reports and radiographic documentation of 12 patients who had been managed operatively for malunion of a fracture of the proximal humerus during the last 8 years. The patients were reviewed 23 months after head preserving surgery with the 90° blade plate and additional bone grafting. Results were rated using the UCLA and Constant-Score. The average age of the patients was 58 years. There were 12 surgical neck non-unions. At time of evaluation ⅔ of the patients were satisfied with the result. 11 out of 12 non-unions healed. Overall 50 % of the patients had very good or good results with the Constant and UCLA Score. In conclusion, blade plating provides a valuable salvage procedure for the difficult reconstructive problem of surgical neck non-unions in the humerus.
