Die Hüftgelenksluxation - Historisches und Aktuelles[1]

Zusammenfassung

Die Hüftgelenksluxation ist eine seltene Verletzung. Sie war aber schon aus biblischer Zeit und aus dem Altertum bekannt. Bereits aus dem Mittelalter gibt es genaue Schilderungen, aus denen bereits die Schwierigkeit bei der Abgrenzung zu hüftnahen Frakturen hervorgeht. Arbeiten aus dem 19. Jahrhundert haben bereits zur Klassifizierung dieser Verletzungen beigetragen. Mit der Röntgendiagnostik verschwanden alle Zweifel an der Diagnose. Notfallmäßig muss die sofortige Reposition in Vollnarkose vorgenommen werden. Unter Kenntnis des Luxationstyps gelingt dies meist problemlos. In der Nachbehandlung sollte das Gelenk länger entlastet werden, mögliche Spätfolgen (Hüftkopfnekrose, Arthrose) erfordern einen längeren Beobachtungszeitraum.

Abstract

The luxation of the hip joint is rare. Since ancient time and from the bible this injury is known. The first exact descriptions from the Middle Ages could not distinguish luxations from fractures of the hip. Publications from the 19th century described the early classifications of those luxations. Since X-ray were discovered there were no more doubt about fractures and hip luxations. The luxation of hip has to be removed in general anaesthetic. With correct knowledge of the direction of hip luxation the reset of the joint is easy. The follow-up treatment contains the protection of the hip to prevent necrosis of the femoral head and early arthritis. We recommend a follow-up for years.
