Heilmittelverordnung und Erbringung nach dem Heilmittelkatalog 2001

Zusammenfassung

Fragestellung: Steigende Ausgaben für Heilmittel in Deutschland veranlassten die Techniker Krankenkasse zu einer Überprüfung verordneter Leistungen. Material und Methode: Der Medizinische Dienst der Krankenversicherung Rheinland-Pfalz überprüfte 482 bei der Krankenkasse konsekutiv eingegangene Verordnungen für Physikalische Therapie aus den Jahren 2003/2004 auf Konformität mit dem Heilmittelkatalog in der Fassung 2001. Ergebnisse: Nicht zu beanstanden waren die Fachgebiete der verordnenden Ärzte, die indikativen Diagnosen und die Auswahl der überwiegend aktivierenden Heilmittel. 84 % der Heilmittel wurden von Orthopäden, Chirurgen und Allgemeinmediziner verordnet. Orthopädische, traumatologische und rheumatologische Diagnosen waren in 88 % der Fälle indikativ. 80 % der Verordnungen waren physiotherapeutische Leistungen. Gruppentherapie wurde in nur 0,4 % der Fälle, Hausbesuche in nur 8,5 % der Fälle verordnet. Die Therapieziele wurden zu 96 % als nachvollziehbar eingestuft, die ärztlichen Verordnungen zu 78 % als voll plausibel und zu 13 % als bedingt plausibel. Konform mit dem Heilmittelkatalog 2001 waren 82 % der Verordnungen. Die Abrechnung der Heilmittel erfolgte in 90 % der Fälle wie vom Arzt verordnet. Schlussfolgerung: Durch eine Steigerung des Anteils der Gruppentherapie ließe sich die Wirtschaftlichkeit erhöhen. Die Rate von 18 % nicht konformer Verordnungen deutet wegen der Komplexität des Heilmittelkataloges 2001 auf einen erhöhten Informations- und Schulungsbedarf hin. Die Neufassung der Heilmittel-Richtlinien HMR 2004 erleichtert die konforme Anwendung.

Abstract

Purpose: Increasing costs for medical physiotherapy, ergotherapy and logopaedics in Germany induced the inspection of their prescription. Materials and Methods: The Medizinische Dienst der Krankenversicherung Rheinland-Pfalz reviewed 482 prescriptions for physical treatment of 2003/2004 in the order of entrance at the health insurance company to find out whether the prescriptions agreed with the valid catalogue for the prescription of physiotherapy, ergotherapy and logopaedics. Results: No objections could be raised to the specialty of the prescriber, the indicative diagnoses and the selection of the activating treatments which were prescribed in most of the cases. 84 % were prescribed by orthopaedics, surgeons and general practitioners. Orthopaedic, traumatologic and rheumatologic diagnosis were indicative in 88 %. 80 % were prescriptions for physiotherapy. Group therapy was prescribed in only 0.4 %. Home visits were prescribed in only 8.5 %. A conformity of the prescriptions with catalogue for the prescription of physiotherapy, ergotherapy and logopaedics could be determined in 82 %. In 90 % the treatment was invoiced according to the prescription. Conclusions: By increasing the quota of group therapy the system would be more economic. The rate of 18 % of nonconformic prescriptions because of the complexity of the catalogue for physical treatment point out the increased need for information and training. Simplifying the catalogue for physical treatment is recommended and had been realised in the new version in 2004.
